28 Don't harm your body just to change its appearance
This section is about a few ways of changing how you look by doing something to the body: going hungry, taking drugs, getting injections, and having surgery. The decision is usually made by comparing yourself with other people. The cost falls entirely on you, and a fair part of it cannot be undone.
This section does not judge what anyone wants to look like. That is each person's own affair. For the same goal, you can look for a prescription, a qualified provider, and follow-up, or you can buy online, go to a beauty salon, and raise the dose yourself. This section only compares how far apart the risks of those 2 ways are. The account looks mainly at what actually happens to the body, such as blindness, a stay in hospital, and death. Those 2 items on cosmetic medicine also involve criminal liability for practicing medicine illegally.
Items in this section (10)
- 28.1 Do not use extreme dieting, fasting, or making yourself vomit to control weight. If you want to lose weight, lose it on the exercise side
- 28.2 Two things to check before getting injections, thread lifts, or surgery: does the facility’s license list “medical aesthetics,” and is the practitioner an attending physician?
- 28.3 For facial fillers, avoid the bridge of the nose, the area between the eyebrows, and the forehead, and do not go to an irregular injector just to save money
- 28.4 Do not buy weight-loss pills, weight-loss coffee, slimming candies, or enzyme plums (plums claimed to have been soaked in enzymes) that promise you will get thin fast
- 28.5 Do not use anabolic steroids (“muscle-building injections” or “oral pills”) to build muscle
- 28.6 If you take a weight-loss drug, or a weight-loss injection such as semaglutide, get the prescription at a hospital and raise the dose and come back for checks as the doctor arranges. Do not buy from an online shop that ships without a prescription, and do not get injected at a beauty salon
- 28.7 Use sex-hormone drugs only when a doctor prescribes them and checks you on a schedule. Do not buy them online, and do not raise the dose yourself
- 28.8 If feeling that you look ugly has reached the point of checking the mirror over and over and thinking over and over about changing it, get a body-image assessment before you talk about surgery
- 28.9 To straighten teeth, have a doctor with orthodontic qualifications examine you in person, and go back on schedule. Do not buy aligners online and wear them at home
- 28.10 Before you buy a spot-fading whitening cream, take the «国妆特字» number on the package and check it on the National Medical Products Administration website. Do not use a spot-fading cream sold by a WeChat reseller, or one the beauty salon mixed itself

People with anorexia nervosa (the illness of starving yourself past the point of health) are about 5.9 times as likely to die as people the same age. Follow 1000 of them for 1 year and 5.1 die, and about 1 in 5 of those deaths are suicide. People who diet are more likely to get an eating disorder. Students at 44 secondary schools were followed for 3 years, and girls who dieted severely were 18 times as likely to get ill as girls who did not diet. If you want to control your weight, exercise more. Do not eat so little that you are hungry.
- Cost
No money. The other way around, you also save what you would have spent on weight-loss products.
- Benefit
36 studies taken together. The people with anorexia nervosa had been followed for 166642 person-years in all (the years each person was followed, added up). Their standardized mortality ratio was 5.86, that is, the chance of death is about 5.9 times that of people the same age. There were 5.1 deaths per 1000 person-years, which means 5.1 deaths if 1000 people are followed for 1 year. About 1/5 of the people who died died by suicide. The standardized mortality ratio for bulimia was 1.93, and for other eating disorders it was 1.92, about 1.9 times people the same age in each case. Another study, in 44 secondary schools in Victoria, Australia, followed students aged 14 to 15 for 3 years, and the same students were checked across 6 rounds. New eating disorders in girls were 21.8 per 1000 person-years, that is, 21.8 new cases if 1000 girls are followed for 1 year. Girls who dieted severely had 18 times the risk of girls who did not diet. Girls who dieted moderately had 5 times the risk. After a further adjustment for past dieting and psychiatric symptoms, taking out the effect of having dieted before and of already having psychiatric symptoms, BMI (body mass index, weight divided by height squared), amount of exercise, and sex no longer predicted a new eating disorder. The authors' conclusion was that "among adolescents, controlling weight with exercise rather than restricting food seems to carry a lower risk of an eating disorder."
- Notes
"Severe dieting" here means keeping what you eat far below what the body needs for a long time. It does not mean skipping 1 meal or skipping a late-night snack. If making yourself vomit, guilt after a binge, or weighing yourself over and over and being unable to stop has already appeared, go to a psychiatry department or the clinical psychology department of a general hospital. This is an illness with a standard treatment. It is not a willpower problem. The link between BMI and mortality is in section 2, item 32. That item is about the long-term weight range, not about the method of losing weight.

A needle, a knife, or a medical device that breaks the skin or goes into the body counts as medical care. It can be done only where the license lists "medical aesthetics," and the person who does it must be a registered licensed physician. Beauty salons, studios, and hotel rooms are not allowed. A person with no physician's qualification who does it is practicing medicine illegally. If they seriously injure someone, the sentence is 3 to 10 years. If they cause death, the sentence is 10 years or more.
- Cost
There is no cost involved. Checking these two documents takes only a few minutes. The clinic should display its “Medical Institution Practice License” in the lobby. A doctor’s qualifications can be verified via the health authority’s licensed-physician lookup.
- Benefit
Article 2 of the “Administrative Measures for Medical Aesthetic Services” defines medical aesthetics as “the use of surgery, drugs, medical devices, and other invasive or traumatic medical techniques to repair and reshape a person’s appearance and body contours.” In simpler terms, any procedure that breaks the skin or introduces needles or devices into the body qualifies as medical treatment. Articles 8 and 24 stipulate that a facility must first obtain a “Medical Institution Practice License” and have medical aesthetics approved as a service subject by the registration authority before conducting such procedures; if “medical aesthetics” is not listed on the license, the practice is prohibited. Article 11 requires attending physicians to hold valid licenses and be registered; those performing cosmetic surgery must have at least six years of relevant clinical experience, cosmetic dentistry five years, and cosmetic traditional Chinese medicine or dermatology three years each. Article 20 requires that, before the procedure, patients be told in writing the indications, contraindications (when it is suitable and when it must not be done), medical risks, and precautions, and that their signed consent be obtained. Article 336 of the Criminal Law criminalizes illegal medical practice — performing injections, surgery, or treatment without proper licensing. Where the circumstances are serious, the penalty is up to 3 years of fixed-term imprisonment, criminal detention, or public surveillance, with or without a fine, or a fine alone. Where the patient's health is seriously harmed, it is 3 to 10 years of imprisonment plus a fine. Where the patient dies, it is 10 years or more of imprisonment plus a fine.
- Notes
Article 16 of the same regulations further specifies that medical aesthetic procedures may only be conducted at licensed medical aesthetic facilities or at hospitals/clinics with designated medical aesthetic departments. Therefore, performing injections at a patient’s home or in a private studio already violates regulations. Article 20 also states that minors and others who cannot yet decide for themselves need a guardian's consent for such treatments, with a parent's signature. Routine beauty services such as facials, massages, or nail care fall outside this regulatory framework; however, any procedure involving needles or surgery remains classified as medical aesthetics.

Of the people who went blind after a filler injection, more than half were injected in the nose, and next came the area between the eyebrows, the forehead, and the folds beside the mouth. More than 8 in 10 were given hyaluronic acid. Of the people who went blind, only 2 in 10 got their sight fully back, a bit more than 1 in 10 got part of it back, and nearly 2 in 10 also had damage to the brain. No rescue method works reliably right now, so avoid these sites beforehand.
- Cost
No money. You only inject a different site, or you do not have it done.
- Benefit
Cases reported in journals around the world, added up. From January 2015 to September 2018 there were 48 new cases of partial or complete loss of vision after a filler injection. The site with the most events was the nose, at 56.3%. Next were the area between the eyebrows at 27.1%, the forehead at 18.8%, and the nasolabial fold (the groove people call laugh lines) at 14.6%. In 81.3% of the cases the filler was hyaluronic acid. 43.8% had skin changes. 18.8% had a complication in the central nervous system (something went wrong in the brain). 10 cases (20.8%) got their vision fully back, 8 cases (16.7%) got part of it back, and the rest did not recover. The authors' conclusion was that the ways different places handled it differed widely, and no treatment has been shown to work consistently.
- Notes
These figures were counted case by case from cases reported in journals. They include only the people who had the event, not the total number of people injected. So you cannot calculate how likely 1 injection is to cause blindness. You can only see which sites have more events, and that afterward it basically cannot be reversed. The evidence is graded B for this reason too. What a regular institution does to lower the risk is already routine. First map how the blood vessels run under the place to be injected. Use a blunt needle rather than a sharp one. Push a small amount while withdrawing the needle. Have hyaluronidase (the enzyme that breaks down hyaluronic acid) ready beforehand. Each of these requires that the person doing it is a trained doctor. That is also the reason not to use an irregular channel. Before you do it, check it against item 2 of this section (check the facility license and the attending physician).

These products are often found with a diet drug (sibutramine) added in secret. It used to be a weight-loss drug, and China stopped it in 2010. Overweight people aged 55 or older, who had heart disease or diabetes, took it for more than 3 years. Non-fatal heart attacks were about 30% higher, and strokes were about 36% higher. The weight lost was only 1.7 kg more. By the time a spot check finds it, you have already swallowed it.
- Cost
No money. You also save what you would have spent on these products. The hard part is accepting that there is no such thing as dropping several jin (a Chinese unit of weight, about half a kilogram) in 1 week.
- Benefit
The SCOUT trial enrolled 9804 people. All were 55 or older, overweight or obese, and had cardiovascular disease or type 2 diabetes. They drew lots and were split into a group that took the drug and a group that did not. Treatment lasted 3.4 years on average. The main endpoint was a set of bad events chosen in advance: non-fatal heart attack, non-fatal stroke, coming back after cardiac arrest, and cardiovascular death. The rate of these events was 11.4% vs 10.0%. HR 1.16 (95% CI 1.03-1.31, P=0.02). The chance of an event was about 16% higher for the people who took the drug. The true value roughly sits between 3% higher and 31% higher. Non-fatal heart attack was 4.1% vs 3.2%, HR 1.28 (1.04-1.57), about 30% higher. Non-fatal stroke was 2.6% vs 1.9%, HR 1.36 (1.04-1.77), about 36% higher. Cardiovascular death and all-cause death did not increase. All-cause means every cause added together. After the draw, the sibutramine group lost 1.7 kg more on average. The former State Food and Drug Administration, on the basis of a safety assessment, held that "the risks of weight-loss treatment outweigh the benefits," and in October 2010 it notified a stop to the production, sale, and use of sibutramine preparations and the raw drug. Both sibutramine and phenolphthalein (a laxative chemical) are prohibited as additives in food, and health food counts as food.
- Notes
SCOUT enrolled a high-risk group who already had heart disease or diabetes. This trial did not prove that healthy young people would have the same events. But because you cannot tell who is high-risk, the regulator simply took it off the market. Another ingredient often added in secret is phenolphthalein. It was originally a laxative. Taking too much of it for a long time can throw off the body's electrolytes and trigger an irregular heartbeat. How to tell: a real weight-loss drug is a prescription drug, and the approval number can be looked up at a pharmacy or a hospital. Something sold under the name of "coffee," "candy," "plums," or a "meal replacement," that also promises you will drop several jin in 1 week, should by definition have no drug effect. If it has a drug effect, something was added. The original text that stopped sibutramine is notice «国食药监办» [2010] No. 432, posted on the National Medical Products Administration website. What is cited here is a page on which the Tianjin market supervision administration retells it. The 2 sentences "risks outweigh the benefits" and "production, sale, and use stopped in October 2010" can be checked word for word there.

28.5 Do not use anabolic steroids (“muscle-building injections” or “oral pills”) to build muscle

Men who test positive on a gym doping check have 3 times the risk of death of people the same age, and they go to the hospital more than twice as often each year. Acne, male breast growth, and trouble getting an erection each hit at least 1 in 10 of the people who use these drugs. In China these drugs are prescription drugs, and a channel that sells them to you without a prescription is itself breaking the law.
- Cost
There is no cost involved; you even save money on medication. The hard part is accepting that muscle growth will be slow.
- Benefit
The Danish researchers went back through records and matched each steroid user with controls of the same age. The original cohort comprised 545 men whose gym doping tests were positive for androgenic steroids between 2006-01-03 and 2018-03-01; they were paired with 5450 controls. An additional validation used 644 refusers and 6440 controls. Results showed a three-fold increase in mortality among users (HR = 3.0; 95 % CI 1.3–7.0). The true multiple sits roughly between 1.3 and 7. The median yearly number of hospital admissions or visits was 0.81 versus 0.36 (P < 0.0001). Median means that half of the users had more visits and half fewer; such a low P-value indicates this difference is highly unlikely to be random. Acne, gynecomastia, and erectile dysfunction each affected over 10 % of users – at least one in ten – a rate clearly above that of controls (P < 0.0001). The validation cohort yielded the same findings. Article 7 of China’s Anti-Doping Regulations states that all substances on the prohibited list are strictly controlled; no organization or individual may produce, sell, import, or export them illegally. The same regulation designates anabolic agents and peptide hormones as prescription drugs, meaning they may be obtained only with a doctor’s prescription; “anabolic agents” is the official term used in legislation.
- Notes
Disputed: This is an observational study, not a randomized trial; steroid users may also engage in other risky behaviors, so the three-fold risk increase may not be attributable solely to the drugs. The true effect size likely falls between 1.3 and 7.0. However, visible side effects such as acne, gynecomastia, and erectile dysfunction are less prone to bias. Legally these products are termed “anabolic agents”; any product described with that phrase contains them. Ordinary protein powder or creatine does not fall under this category. A problem is that some products marketed as “muscle-building powders” or “nutritional supplements” may contain secretly added ingredients while claiming especially fast muscle growth. The screening method is the same as for weight-loss products (see item 4: avoid any weight-loss drug promising quick results). Legitimate drugs are prescription-only and carry verifiable approval numbers; if a product sold as a “muscle-building powder” or “nutritional supplement” promises results within weeks and really works, something was added to it.

A prescription drug has to be sold against a prescription. Selling it online also takes a real prescription and a real name. An online shop that ships without a consultation, or a beauty salon that comes to your home to inject, is usually supplying drugs of unclear origin, and nobody is responsible for the dose or for who should not have it. 1 year after the weight-loss injections stop, about 2/3 of the weight lost comes back, so count it as long-term treatment.
- Cost
1 registration visit, plus the trip there and back. Tens of yuan, and half a day. If you are getting the weight-loss injections, you also go back for checks at the times the doctor sets. The money for the drug is separate. After you stop the injections the weight mostly comes back, so count this money as long-term use.
- Benefit
The Measures for the Supervision and Administration of Drug Distribution and Use Quality say that a drug retailer must follow the system that sorts drugs into prescription drugs and non-prescription drugs. That system puts drugs in 2 kinds. One kind can be bought only with a doctor's prescription. The other can be bought by the person. The pharmacy must sell prescription drugs against a prescription, as the rules require, and keep the prescription for at least 5 years. When the pharmacist or another pharmacy technician is not on duty, a sign must say so, and prescription drugs must not be sold without their review. Prescription drugs must not be sold from open shelves, for a customer to take down. Article 9 of the Measures for the Supervision and Administration of Online Drug Sales says that, when prescription drugs are sold to an individual online, the seller must make sure the prescription comes from a real and reliable source, and must use real-name rules. Article 10 requires a risk warning shown prominently under each drug's display page. Before the prescription has been reviewed, the package insert must not be shown, and no buying service may be offered. Before the prescription is reviewed, the page should not even give a button to buy. Article 8 says that drugs under special state control must not be sold online. These include vaccines, blood products, narcotic drugs, psychotropic drugs, toxic drugs for medical use, radioactive drugs, and drug-class precursor chemicals. The National Health Commission's Guidelines for the Diagnosis and Treatment of Obesity (2024 edition) state that 5 drugs have been approved by the National Medical Products Administration for weight loss in adult primary obesity. The 5 are orlistat, liraglutide, beinaglutide, semaglutide, and tirzepatide. The same guidelines count a BMI (body mass index) from 24 up to but not including 28 as overweight, and 28 and above as obese. A drug is considered only when the person is overweight and also has a related disease, such as high blood sugar, hypertension (high blood pressure), abnormal blood lipids, or fatty liver, or is already obese. The premise is that changing how you live has not brought the weight down. The drug is added on top of that. The guidelines require a full assessment of the illness and the contraindications before the drug, and regular follow-up under a specialist physician while it is used. Semaglutide starts at 0.25 mg a week, with a maintenance dose of 2.4 mg or 1.7 mg. Tirzepatide starts at 2.5 mg a week, up to 15 mg. If the weight lost is under 5%, the guidelines suggest stopping the drug. What happens after stopping is in 3 trials. The STEP 1 extension followed 327 people. After 68 weeks on semaglutide they had lost 17.3% on average. 1 year after stopping, the weight had come back by 11.6 percentage points, and the net loss left was 5.6%. The authors' conclusion was that they had regained 2/3 of the weight previously lost, and that most cardiovascular and metabolic markers had also moved back near the start. STEP 4 took 803 people who had already injected for 20 weeks and lost 10.6% on average, and split them by drawing lots. Those who kept injecting lost another 7.9% over the next 48 weeks. Those switched to a placebo regained 6.9%. In SURMOUNT-4, 670 people first injected tirzepatide for 36 weeks and lost 20.9% on average. Those who kept injecting then lost another 5.5%. Those switched to a placebo regained 14.0%. In July 2026 the Yinchuan market supervision administration issued a risk alert. It said that the internet-famous "weight-loss injections" and "slimming pens" on the market are mostly repackaged drugs of unknown source, smuggled drugs, or counterfeit drugs. Buying them online or through a purchasing agent and injecting them yourself can easily set off a severe allergy, acute pancreatitis, organ damage, and a wound infection. An injection of this cosmetic-medicine kind can be done only at an institution that has a medical-aesthetics qualification. An injection at the person's home, or a private injection at a beauty salon, is not compliant. See item 2 of this section (2 things to check before injections, thread lifts, or surgery).
- Notes
The articles can be checked word for word, and the figures for weight coming back after stopping come from trials that split people by drawing lots. But nobody has quantified "how many fewer events you get if you use a prescription instead of buying it yourself," so it is graded B as hard to quantify. The STEP 1 extension stopped not only the drug. The guidance on diet and exercise stopped with it, and only some of the people in the original trial were followed. The steroids and sex hormones in items 5 and 7 of this section follow the same rules. When weight really has to come off with a drug, the doctor first looks at BMI, at whether there are other diseases, and at whether there is a reason the drug cannot be used, and only then decides whether to take it and which one. That judgment cannot be bought.

This does not discuss whether to use them. It only discusses how. Transgender women who use estrogen have clearly more blood clots in the vessels than people the same age, about 4 extra per 1000 people over 2 years, and about 17 extra over 8 years. This kind of risk has to be watched with regular follow-up. If you buy the drugs online and set the dose yourself, nobody is checking you.
- Cost
Regular registration visits, plus blood tests. A few hundred yuan a year, and a few half-days.
- Benefit
The researchers followed a group in the electronic records of Kaiser Permanente, a health-care group. There were 2842 transgender women and 2118 transgender men. Average follow-up was 4.0 years and 3.6 years. Each person was matched, on birth year, race, the clinic, and the time of joining, with about 10 times as many cisgender controls, 48686 cisgender men and 48775 cisgender women in all. Transgender women had more venous thromboembolism (a blood clot blocking a vessel) than the controls. Compared with cisgender men, at 2 years it was 4.1 cases per 1000 people (95% CI 1.6-6.7), and at 8 years it was 16.7 cases (6.4-27.5). Counted out, that is 4.1 extra cases among 1000 people over 2 years, and 16.7 extra over 8 years. Compared with cisgender women, the 2 figures were 3.4 (1.1-5.6) and 13.7 (4.1-22.7). Ischemic stroke (the kind of stroke in which a vessel is blocked) and myocardial infarction were, overall, close across the groups. Among transgender women who started hormone treatment only during the follow-up, the differences in venous thrombosis and ischemic stroke were clearer. The evidence on transgender men is not enough for a conclusion. The authors' conclusion was that "these results may suggest a need for long-term vigilance about the vascular side effects of estrogen in transgender hormone treatment."
- Notes
The researchers themselves wrote down the shortcoming of this study. They could not find out how many people had bought the drugs themselves outside the hospital system. So the line of whether a person was on the drug was never drawn cleanly. That, in turn, is why drug use should sit inside regular follow-up. This study only looked at whether the blood vessels had an event. It does not answer whether hormone treatment should be used, or what good it does. That is a different set of evidence. Every use of sex hormones is the same, including contraceptive pills, menopausal hormone treatment, and testosterone supplementation. If you buy it online yourself, adjust the dose from posts on the internet, and never check clots or liver and kidney markers, the risk sits in the stretch where nobody is watching, not in the reason for using the drug. The regular path in China is the endocrinology department, or the matching specialty, of a proper medical institution, for an assessment, a prescription, and follow-up.

About 15% of people seen in plastic surgery have body dysmorphic disorder, which means magnifying a flaw other people can barely see until it affects daily life. About 13% of people seen in dermatology have it. In both groups, more than 70% are women. Book 1 visit first, see whether this is your situation, and only then decide whether to have surgery.
- Cost
Register for 1 visit at a psychiatry department or a clinical psychology department. Tens of yuan. The hard part is making this trip before you book the surgery.
- Benefit
33 studies were found by the rules and then combined. Among people seen in plastic surgery, the rate of body dysmorphic disorder (magnifying a flaw other people can barely see until it affects daily life) was 15.04%, and the studies ranged from 2.21% to 56.67%. In headcount, about 15 of every 100 people who go for plastic surgery meet it, and study by study the count runs from 2 to 57. Their mean age was 34.54±12.41 years. The second figure is how far the ages spread up and down. 74.38% were women. Among people seen in dermatology the rate was 12.65%, and the studies ranged from 4.52% to 35.16%. Mean age was 27.79±9.03 years, and 76.09% were women. The authors suggest that plastic surgeons and dermatologists assess on their own initiative, pick out the high-risk people, and have several departments manage them together. Another study asked 200 patients with body dysmorphic disorder. 71.0% had sought non-psychiatric treatment such as dermatology or plastic surgery, and 64.0% had received it. The authors' conclusion was that this kind of treatment "rarely improves" body dysmorphic disorder, and looks ineffective for it.
- Notes
The proportions measured in the studies run from 2.21% all the way to 56.67%. The gap is very wide. The reason is that the questionnaires and the groups of people surveyed were not the same. So all you can read from it is that the proportion is not low, and that it is worth screening once first. You cannot treat 15.04% as an exact figure. The grade is B for this reason too. At this proportion, most people who go to plastic surgery do not have body dysmorphic disorder. This item is aimed at the small group in whom it is repeated, ongoing, and already affecting work and social life. Surgery and skin treatment rarely make them better. Clues listed by the British NHS include spending a lot of time comparing your looks with other people, checking the mirror all the time or avoiding mirrors entirely, going to great lengths to cover a body part, and picking at the skin to make it "smooth." It affects work, social life, and relationships.

28.9 To straighten teeth, have a doctor with orthodontic qualifications examine you in person, and go back on schedule. Do not buy aligners online and wear them at home
- Money Low
- Time Hours
- Willpower Some
- Benefit Small
- Measured in Lifespan
- In plain terms
Clear aligners are a Class II (2) medical device in China. The instructions require that you wear them under the guidance of a doctor with orthodontic qualifications, and that you go back regularly. The US drug regulator says it has not approved aligners sold straight to a person without a dentist's prescription. With nobody examining you in person, the bite can go crooked, teeth can move to the wrong place, there can be pain, and teeth can even fall out.
- Cost
It costs more than buying online, for the registration and the return visits, and every so often you have to go to the clinic. The hard part is that the course often lasts 1 or 2 years, and you have to keep the return visits.
- Benefit
The 2022 Guiding Principles for the Registration Review of Bracketless Aligners, from the Center for Medical Device Evaluation of the National Medical Products Administration, say that this kind of aligner has a "management category of Class II (2)." They require the instructions to state that "during treatment it must be worn under the guidance of a physician qualified in orthodontics," and also to state "the requirements for eating, brushing, and regular return visits." On the product name, they say it is "not recommended to contain empty, hypothetical conceptual names such as 'invisible'." The US Food and Drug Administration (FDA) says: "The FDA has not authorized clear aligners that are sold directly to consumers without a prescription and supervision by a dentist or orthodontist," meaning it has not approved a single clear aligner sold directly to a person without a dentist's prescription and supervision. The FDA also says: "In-person examination by a dentist or orthodontist can help lower the risks of uneven bite, unintended tooth movement, pain, and tooth loss," that is, an examination in person by a dentist or an orthodontist can lower the risk of an uneven bite, teeth moving where they should not, pain, and losing a tooth. After you start wearing them, only regular return visits in person can confirm that the teeth are moving safely as planned. Both of these are regulatory documents. Neither gives the proportion of people who come to harm.
- Notes
The benefit magnitude is set at small, because no proportion of harm was found, so the judgment can only rest on the consequences. No risk alert from China's drug regulator or health departments, aimed at buying aligners online and wearing them at home, was found, so this item relies mainly on the registration requirements and on what the FDA says. Orthodontics is a medical act. For tooth straightening done as cosmetic medicine, the institution's license has to list the matching subject, and the doctor who does it has to be qualified. How to check is in item 2 of this section (the institution's license, and the attending physician). In 2023, 4 departments including the National Health Commission issued a notice on dental care. Where there is no practice license or clinic filing certificate, or non-health technicians are hired to treat teeth, the notice requires that they "be severely punished according to the law and the rules." Orthodontics is not reimbursed by yibao (医保 — basic medical insurance). See section 24, item 12 (ask the package price before you get a tooth implant).

28.10 Before you buy a spot-fading whitening cream, take the «国妆特字» number on the package and check it on the National Medical Products Administration website. Do not use a spot-fading cream sold by a WeChat reseller, or one the beauty salon mixed itself
- Money No cost
- Time Minutes

- Benefit Medium

- In plain terms
A spot-fading whitening cream has to be registered with the National Medical Products Administration and get a «国妆特字» number before it can be sold. Spot checks have found spot-fading creams with mercury thousands of times over the limit. The package printed a «国妆特字» number, but the factory named on it had not made that batch. Mercury taken into the body from the face can harm the kidneys. Stopping the cream, plus treatment, gets most people better, but the treatment takes several months to 2 years.
- Cost
No money. Checking a number takes a few minutes. The hard part is not buying the ones that acquaintances and WeChat resellers (微商 — people selling through private chats) push the hardest.
- Benefit
Article 16 of the Regulations on the Supervision and Administration of Cosmetics lists spot-fading and whitening cosmetics as special cosmetics. Article 17 says that special cosmetics may be produced or imported only after registration with the drug supervision department of the State Council. Article 36 requires the label to carry the special-cosmetics registration certificate number. Article 20 requires the registration information to be made public, so the number can be looked up on the National Medical Products Administration website. Article 62 of the Measures for the Administration of Cosmetic Registration and Filing says that a domestic number is «国妆特字» plus the year and a serial number, and an imported number is «国妆特进字». National Medical Products Administration Announcement No. 14 of 2026 published 2 batches of spot-fading cream, in which mercury was found at 4938 mg/kg (milligrams per kilogram) and 9281 mg/kg. The limit listed in the annex to the announcement is not more than 1 mg/kg. These 2 batches were thousands of times over that limit. They were bought at 2 small cosmetics shops, in Sichuan and in Anhui. The package printed a «国妆特字» number. The factory named on it objected. After review, the Guangdong medical products administration found that this enterprise had not produced that batch. A review brought together 30 cases reported in journals, all of them nephrotic syndrome after using a mercury-containing whitening cream, which means a large amount of protein leaking into the urine. All 30 were Asian women, mostly aged 18 to 52. From starting use to falling ill was 1 to 60 months, and half were within 5 months. Most kidney biopsies were minimal change disease (67%) or membranous nephropathy (23%). These are 2 diseases of the glomeruli, the kidney's filters. Very few got better on their own within a month and a half after stopping. In the cases with a record, those who used a mercury-removing drug took 1 to 9 months for the urine protein to turn negative, with a median of 2 months. All 3 who did not use a mercury-removing drug took 12 months or more. A hospital in Beijing admitted 288 people with mercury poisoning from 2014 to 2019. 84 of them (29.27%) were related to cosmetics. The main source of poisoning among women was cosmetics. Of the 18 patients with nephrotic syndrome who were followed, 15 had urine protein and albumin back to normal after an average of 22.67 months. The mercury in these 18 people did not all come from cosmetics.
- Notes
Those 2 batches show that a number can be misused too. When you check the number, see whether the product name, the factory, and the package match what comes up. It is better to buy from the brand's official shop or a regular store. Article 42 of the regulations says that a beauty salon using cosmetics on a customer must follow the rules for a cosmetic operator. Item 5 of Article 60 prohibits an operator from compounding cosmetics on their own. A "salon-line spot-fading cream" that the salon mixes itself is illegal. If you have already used a spot-fading cream of unknown origin for a long time, stop, take the product to a hospital, and check urine mercury and urine protein. In that review, 6 people had normal blood mercury, and all 24 whose urine mercury was checked were above the normal upper limit. The cases were reported case by case, so you cannot calculate what share of people who use a mercury cream will harm their kidneys, and the grade is B for that reason. The benefit magnitude is set at medium from the consequences: nephrotic syndrome can mostly be treated, but treatment takes several months to 2 years.
