27 Pregnancy and childbirth: From confirming pregnancy to discharge and paperwork

The first half of this section measures death risk for the pregnant woman and the fetus. The second half measures money and the time spent running errands. The two accounts are not converted into each other. The content only covers “what to do when.” Things without hard evidence, or with only a small effect — prenatal nutrition, fetal education, and the like — are not written here. Maternity allowances, maternity leave, and the money side of raising a child are in Section 18; how to care for the child after birth is in Section 20; emergencies are in Section 13.

Figures from the World Health Organization: in 2023, about 260000 women worldwide died from pregnancy and childbirth. About 75% of those deaths came from five causes: postpartum hemorrhage, infection, hypertensive disorders of pregnancy (a kind of illness in which blood pressure rises once a woman is pregnant, including preeclampsia and eclampsia), complications during delivery, and unsafe abortion. The WHO also says that “most maternal deaths are preventable, because the health-care solutions to prevent or manage complications are well known.” The items in this section with the largest effect are exactly those that catch these problems early.

Items in this section are grouped by theme below; numbers in parentheses are the item numbers.

What you can do yourself before and during pregnancy: start folic acid when you are preparing to conceive (Item 1); no cigarette and no drink of alcohol (Item 4); you do not need to rehome the cat (Item 17); inactivated influenza vaccine (Item 18); from 28 weeks onward, lie on your side when you fall asleep (Item 19).

Prenatal care and screening: register for the Maternal and Child Health Handbook (Item 2); HIV, syphilis, and hepatitis B (Item 3); low-dose aspirin (Item 5); gestational diabetes screening (Item 6).

Go to the hospital at once when something is wrong: the “go to the hospital immediately” list (Item 7); if your water breaks, lie flat where you are (Item 8).

During childbirth: if you want labor analgesia, say so directly (Item 9); do not ask for a cesarean on your own initiative (Item 10).

After birth: the 42-day postpartum checkup (Item 16); breastfeeding is not the same as contraception (Item 20).

Money and documents: confirm maternity insurance (Item 11); the Medical Certificate of Birth (Item 12); heel-prick screening and hearing screening (Item 13); enroll the child in resident yibao (医保 — basic medical insurance) (Item 14); file the birth registration (Item 15).

Items in this section (20)

  13. 27.13 Do not refuse newborn heel-prick screening or hearing screening
  14. 27.14 Enroll your newborn in yibao right after getting the birth certificate — don’t wait until household registration is complete
  15. 27.15 Within one month after the child is born, go to the police station to file the birth registration
  16. 27.16 Do not skip the 42-day postpartum checkup; in some places it is also a postpartum depression screen
  17. 27.17 You do not need to rehome the cat when pregnant; have someone else scoop the litter every day, cook meat through, and wear gloves when you handle soil
  18. 27.18 If flu season falls during pregnancy, get one shot of inactivated influenza vaccine; any stage of pregnancy is fine; do not use the nasal spray
  19. 27.19 From 28 weeks of pregnancy onward, fall asleep on your side — left or right is fine; do not fall asleep on your back
  20. 27.20 Once you resume sex after birth, use contraception — breastfeeding is not contraception; if you want another pregnancy, wait at least 18 months

In plain terms

Taking one folic acid pill a day around the time of conception cuts the chance of a neural tube defect in the child by about seven tenths. Neural tube defects are birth defects such as anencephaly and spina bifida. For someone who has already had a child with one of these, the chance that another child has the same problem falls to about one third of what it was.

Cost

A few dozen yuan per bottle. Rural women need not pay; the national free folic acid supplementation program issues it. One pill a day; the hard part is remembering every day

Benefit

A Cochrane systematic review pooled 5 trials and 6708 births and rated the evidence high quality. Daily folic acid before and during pregnancy cut fetal neural tube defect risk to RR 0.31 (95% CI 0.17 to 0.58, the plausible range), about a seven-tenths reduction. For people who had already had a child with a neural tube defect, the risk of another affected child was RR 0.34 (95% CI 0.18 to 0.64), about one third of the original. By subgroup, 0.4 mg and higher doses did not differ in effect. Folic acid alone and a multivitamin did not differ either. The Ministry of Health’s 2010 folic acid supplementation program plan states that rural women preparing to conceive receive free folic acid, “taken from 3 months before pregnancy through the first 3 months of pregnancy,” as 0.4 mg tablets. The US Centers for Disease Control and Prevention notes that neural tube defects form in the first few weeks of pregnancy, often before you know you are pregnant

Notes

The key is to start before pregnancy. Neural tube defects form in the first few weeks; if you wait until a positive test, it may already be late. So if you plan to have a child, start 3 months ahead, as the domestic program does. The same pooled review found no clear effect of folic acid on cleft lip and palate, congenital heart disease, or miscarriage — do not count on it for those. Pregnant people on a fully plant-based diet also need vitamin B12 every day; see Section 20, item 14 (if you eat plant-based, take vitamin B12 daily). If you are still undecided about when to have a child, the age account is in Section 18, item 7 (waiting a few more years to have a child).

27.2 Register for the Maternal and Child Health Handbook at a community health center before 13 weeks of pregnancy to claim free prenatal checkups

  • Money No cost
  • Time Hours
  • Willpower None
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

You are entitled to at least five free prenatal checkups during pregnancy, plus home visits after delivery and a 42-day postpartum checkup, all paid for by the state. To access these services, you must register for the Maternal and Child Health Handbook at a community health center before 13 weeks of gestation. Failing to do so means you lose out on all these free services.

Cost

No cost at all — this is part of the national basic public health service program. You’ll need to make several visits based on your gestational age.

Benefit

The third edition of the National Basic Public Health Service Standards outlines specific maternal health management services available to all permanent residents who are pregnant. These include one health checkup in early pregnancy, two in mid-pregnancy, two in late pregnancy, one postpartum home visit, and one 42-day postpartum checkup. Registration for the handbook must be completed before 13 weeks of pregnancy, at which point the first prenatal checkup is also done. The two mid-pregnancy checkups occur between 16–20 weeks and 21–24 weeks respectively, while the two late-pregnancy checkups are scheduled for 28–36 weeks and 37–40 weeks. Local service lists also include a full set of initial lab tests and postpartum depression screening.

Notes

These five free checkups represent the minimum number of services provided; they do not cover all possible prenatal tests. Depending on your individual health needs, your doctor may recommend additional ultrasounds, blood glucose tests, or fetal heart rate monitoring, all of which will incur extra costs. Registering for the handbook as early as possible also allows for earlier assessment of your pregnancy risk level. This risk grading system categorizes pregnant women based on their individual risk factors, ensuring those identified as high-risk receive timely care at specialized medical facilities. Even if you live in a different district than your registered household location, you can still register at your current place of residence — there is no requirement to return to your place of household registration.

27.3 At the first prenatal visit, get the three tests for HIV, syphilis, and hepatitis B; if HIV is found, free prevention of mother-to-child transmission is available

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Big
  • Measured in Lifespan
Value: Very highEvidence A

For Pregnant or planning

In plain terms

At the first prenatal visit, get the three tests for HIV, syphilis, and hepatitis B done. The initial HIV screen does not come out of your pocket. A positive result is not a dead end. If HIV is found, the state provides free treatment and counseling. If syphilis is found, treat early with penicillin. If hepatitis B is found, the newborn gets vaccine and immunoglobulin at the same time at birth, which blocks transmission from mother to child.

Cost

HIV counseling and initial screening are free under the regulations. For syphilis and hepatitis B, national documents do not say they are free; ask the prenatal hospital whether they charge

Benefit

Mother-to-child transmission means the disease passing from mother to child. The Work Specifications for Preventing Mother-to-Child Transmission of HIV, Syphilis, and Hepatitis B (2020 Edition) set requirements. At the first prenatal checkup, the hospital must give relevant information, “provide appropriate, standardized testing, and provide follow-up counseling based on the results.” If the status is still unclear before delivery, treat the person as infected in principle. The specifications require that pregnant women found to have syphilis “receive standardized penicillin treatment as early as possible,” and that the newborn get preventive penicillin right after birth. For pregnant women positive for hepatitis B surface antigen, those who meet the criteria may receive antiviral treatment after informed consent. A positive result is not a dead end. Article 23 of the Regulations on the Prevention and Control of HIV/AIDS states that people who voluntarily accept HIV counseling and testing are “provided free counseling and initial screening.” Article 44 requires free “treatment and counseling to prevent mother-to-child transmission of HIV” for pregnant women infected with HIV. Article 43 requires services such as blocking transmission (keeping the disease from the child), treatment, postpartum visits, and infant follow-up and testing. When the mother is positive for hepatitis B surface antigen, the newborn must receive hepatitis B vaccine and hepatitis B immunoglobulin at the same time at birth (see Section 20, item 2)

Notes

The earlier these three are checked, the more useful they are, because blocking and treatment both take time. Article 3 of the HIV/AIDS regulations also states that “no unit or individual may discriminate against people living with HIV, AIDS patients, or their family members”; lawful rights in marriage, work, medical care, and schooling are protected. Everyday prevention and testing are in Section 1. Post-exposure prophylaxis after high-risk behavior is in Section 13

27.4 Through the whole pregnancy, do not take a single cigarette or a single drink of alcohol, and do not let others smoke indoors at home either

  • Money No cost
  • Time Minutes
  • Willpower A lot
  • Benefit Big
  • Measured in Lifespan
Value: HighEvidence A

For Pregnant or planning

In plain terms

Smoking in pregnancy raises the chance the baby is born early, is born too small, or has a cleft lip or palate, and doubles the risk of abnormal bleeding in pregnancy and at delivery. When the pregnant person smokes, or the baby breathes smoke after birth, the risk of sudden infant death is higher. Quitting is still useful whenever you do it.

Cost

No money spent, and you save on tobacco and alcohol. The hard part is getting people who live with you to quit along with you

Benefit

The US Centers for Disease Control and Prevention’s framing: smoking in pregnancy can leave a baby “born too small, even if full term” and “may lead to preterm birth.” It “doubles the risk of abnormal bleeding during pregnancy and delivery.” It raises the risk of birth defects such as cleft lip and cleft palate. It also “can damage a baby’s developing lungs and brain,” and “the damage can last into childhood.” Infants of mothers who smoked in pregnancy, and infants exposed to smoke after birth, have a higher risk of sudden infant death syndrome. Official advice is to “quit smoking before getting pregnant if you can, but quitting is still helpful if you are already pregnant” and “it is never too late to quit.” On alcohol, the CDC says there is “no known safe amount of alcohol use during pregnancy,” and no stage of pregnancy when drinking is safe. The baby’s brain develops through the whole pregnancy, so alcohol contact at any time may cause harm

Notes

Secondhand smoke when someone else smokes indoors counts the same for the fetus. So people who live with you should not smoke indoors either. Alcohol is the same: no known safe amount; the simplest approach in pregnancy is not to drink at all. The account of tobacco and alcohol for adults themselves is in Section 2

27.5 For women at high risk of preeclampsia, start one low-dose aspirin tablet daily after week 12 of pregnancy, as your doctor directs

  • Money Low
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

Preeclampsia is a kind of high blood pressure that occurs in pregnancy. You count as high risk if you had it in your last pregnancy, are carrying twins or more, have chronic high blood pressure, had diabetes before the pregnancy, or have kidney disease or an autoimmune disease. If you are high risk, take one low-dose aspirin tablet a day from week 12 as your doctor directs. Preeclampsia is about 15% lower, and premature birth and the baby dying around the time of birth are each about 20% lower. Everyone else should not take it on their own.

Cost

A few dozen yuan. Take the medicine every day as your doctor directs, all the way to delivery. The hard part is remembering to take it every single day.

Benefit

The US Preventive Services Task Force gives this a Grade B recommendation: for people at high risk of preeclampsia, start low-dose aspirin (81 mg per day) after week 12 of pregnancy to prevent it. Preeclampsia is a hypertensive disorder of pregnancy. Combining many studies gives the results below; the 95% CI is the range of confidence. Preeclampsia: RR 0.85 (95% CI 0.75 to 0.95, 16 studies), about 15% lower. Premature birth: RR 0.80 (95% CI 0.67 to 0.95, 13 studies), about 20% lower. Perinatal death: RR 0.79 (95% CI 0.66 to 0.96, 11 studies), about 21% lower; perinatal death means death in the period around birth. Small for gestational age / intrauterine growth restriction: RR 0.82 (95% CI 0.68 to 0.99, 16 studies), about 18% lower; this means the fetus grows smaller than others of the same gestational age

Notes

Those counted as high risk are: preeclampsia in the last pregnancy, a twin or multiple pregnancy, and chronic high blood pressure. Also type 1 or type 2 diabetes before the pregnancy, kidney disease, and autoimmune diseases such as systemic lupus erythematosus or antiphospholipid syndrome. If you are not in these groups, do not take it on your own. This drug needs a doctor's prescription, so give your full history at the prenatal checkup and let the doctor decide. Most of what is sold in China is a 100 mg enteric-coated tablet; take it as your doctor directs

27.6 At 24 weeks of pregnancy or later, get one gestational diabetes screen; do not skip it because drinking the sugar water is a bother

  • Money Low
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

Gestational diabetes is high blood sugar that appears only after pregnancy begins. After 24 weeks, half a morning of drinking sugar water and several blood draws can find it. If you neither screen nor manage it, the risks of a fetus that grows too large, difficult birth, a cesarean, preeclampsia, and newborn low blood sugar all go up.

Cost

A few dozen to a little over 100 yuan. You go fasting, several blood draws, and it takes half a morning

Benefit

The US Preventive Services Task Force gives a Grade B recommendation: screen pregnant people without symptoms (no felt discomfort) for gestational diabetes at 24 weeks or later. Gestational diabetes is high blood sugar that appears only after pregnancy begins. When only one screen is done, that one should be at 24 weeks or later. On screening before 24 weeks, the Task Force concludes that current evidence is insufficient to judge benefits and harms (Grade I). Untreated gestational diabetes raises the risks of macrosomia (a fetus that grows too large), difficult birth, cesarean, preeclampsia, and newborn hypoglycemia

Notes

Per the US Preventive Services Task Force, after a diagnosis people usually start with exercise, diet changes, and blood sugar checks; insulin or oral drugs come only if sugar stays uncontrolled. Do not skip this screen because you fear needing injections. Recheck blood sugar after birth: the US Centers for Disease Control and Prevention says about half of people who had gestational diabetes later develop type 2 diabetes

27.7 Memorize this “go to the hospital immediately” checklist — it applies throughout pregnancy and the first year after childbirth

  • Money No cost
  • Time Minutes

  • Benefit Big
  • Measured in Lifespan
Value: Very highEvidence A

For Pregnant or planning

In plain terms

If any of the following appears, go to the hospital the same day — do not wait overnight at home; this still counts within the first year after birth. A headache that will not ease or keeps getting worse, vision that looks different, fever of 38 °C, severe swelling of the hands or face. Trouble breathing, chest pain or a fast heartbeat, severe abdominal pain that does not ease, fetal movement that stops or drops clearly. Vaginal bleeding or fluid leaking, one arm or leg that is both swollen and painful, thoughts of harming yourself or the baby.

Cost

No cost at all. It takes just five minutes to memorize, and it’s a good idea to share it with anyone living in the same household.

Benefit

These are all emergency warning signs during pregnancy and postpartum listed by the US Centers for Disease Control and Prevention. Persistent or worsening headaches, dizziness or fainting, changes in vision, a body temperature of 38°C or higher. Severe swelling of the hands or face, thoughts of harming yourself or your baby. Difficulty breathing, chest pain or a rapid heartbeat. Severe nausea and vomiting, unrelenting severe abdominal pain. Absence or significant reduction of fetal movement during pregnancy, vaginal bleeding or fluid leakage during pregnancy. Abnormal vaginal bleeding or discharge after childbirth. Severe swelling, redness, or pain in one arm or leg. These problems can all occur within the first year after childbirth. It is not the case that all is well once the postpartum month (月子) is over.

Notes

This checklist serves to clearly distinguish between situations where you can wait a little longer and those that demand immediate medical attention. Headaches paired with blurred vision and upper abdominal pain point to preeclampsia, a form of pregnancy-related hypertension. Swelling and pain in one lower leg often indicates deep vein thrombosis (see Section 13). According to the World Health Organization, severe postpartum hemorrhage can be fatal to a healthy woman within just a few hours. If any of these symptoms appear, go to the hospital right away — do not delay by waiting at home overnight.

27.8 If your water breaks, lie flat where you are, raise your hips, and call 120; do not walk around and do not bathe

  • Money No cost

  • Benefit Big
  • Measured in Lifespan
Value: Very highEvidence B

For Pregnant or planning

In plain terms

When your water breaks, do not pack slowly and then leave. The right move is to lie down where you are, raise your hips, and call 120. Walking while standing makes the fluid drain faster, and the cord may come out with it. That is an emergency that can kill the baby.

Cost

No money spent

Benefit

The Beijing Municipal Health Commission’s handling advice: after the water breaks, “immediately find a place to lie flat to stop the amniotic fluid from flowing out,” “do not walk around casually,” and “call 120 emergency services at once.” While waiting for the vehicle, tell family to bring things and meet at the hospital. Medical explainers add why: when a person stands, amniotic fluid is lost faster, and the baby’s umbilical cord may come out with it — cord prolapse, an emergency that threatens the fetus. So lie down, raise the hips; a left lateral position is fine

Notes

The three signs that birth is near are regular contractions, show (bloody mucus), and water breaking; water breaking is the one that means “lie down on the spot and call a car.” This is the author’s summary of the two explainers above. An explainer from Shunyi District Maternal and Child Health Hospital says getting out of bed to stand, walk, sit, or squat all make fluid drain faster and raise the risk of cord prolapse. So after the water breaks, do not bathe, do not squat on the toilet, and do not drive yourself. Note the time the water broke and the color of the fluid, and tell the doctor. Fluid that is light green, yellow-green, or even brownish yellow may mean meconium (the baby’s stool) is mixed in. If you feel a cord-like object hanging in the vagina, it may be cord prolapse — call 120 at once, lie flat, and raise the hips. If fetal movement is clearly different from usual, tell the doctor as well

27.9 If you want labor analgesia, say so directly. It does not raise the risk of C-section

  • Money Low
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Time
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

Labor analgesia is a shot of anesthetic in the lower back that takes the edge off labor pain. It works better than other pain relief and does not make a cesarean more likely. The claim that “it leaves you with lifelong back pain” has no basis either. It now has its own billing item. The aim is to make hospitals more willing to offer it.

Cost

Several hundred to over 1,000 yuan; prices vary by region. Be sure to ask the hospital and anesthesiology department in advance whether this service is available. The main hurdle is simply bringing it up yourself.

Benefit

A Cochrane systematic review found that epidural analgesia is more effective than non-epidural methods at reducing labor pain and improving maternal satisfaction. Labor analgesia uses exactly this technique. The same review states that epidural analgesia has no effect on the risk of C-section or long-term back pain. Instrumental delivery — using tools to assist birth — shows no increased incidence in studies published after 2005. Starting in 2024, the National Healthcare Security Administration introduced separate pricing categories for “labor analgesia,” “doula-assisted childbirth,” and “family-member labor support,” precisely because previously there were no dedicated rates to motivate hospitals to offer these services.

Notes

Both popular myths — “epidural analgesia causes lifelong back pain” and “it causes a difficult labor and a C-section” — lack any scientific basis. Cochrane’s comparison of epidural analgesia with other pain-relief methods confirms it has no impact on C-section risk or chronic back problems. Real side effects may include low blood pressure, fever, weak legs, difficulty urinating, longer first- and second-stage labor, and a higher need for oxytocin. The decision to use it is yours; don’t let unfounded rumors deter you. Availability also depends on whether the hospital has an anesthesiologist on duty 24/7 — be sure to ask during prenatal visits.

27.10 Without a medical indication, do not ask for a cesarean, and do not schedule surgery to pick a date

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

The World Health Organization’s position is that once a place’s cesarean rate passes 10%, there is no evidence that deaths of mothers and newborns keep falling. When a cesarean is needed, it must be done. Choosing a date or avoiding pain as the reason means taking on one more major abdominal operation.

Cost

No money spent. The hard part is standing up to what people at home say

Benefit

WHO statement in 2015: when the cesarean rate in a population rises to about 10%, maternal and newborn deaths fall. Above 10%, there is no evidence that mortality improves further. WHO also stresses that every effort should be made to provide cesareans to women who need them, not to hit a particular rate

Notes

When medicine truly requires it, a cesarean is life-saving — do it; do not tough it out. What to avoid is surgery when medicine does not require it, for example fear of pain, picking an auspicious time, or gaming the child’s school-entry age. A cesarean is a major abdominal operation. One pooled comparison of cesarean with vaginal birth found that after a cesarean, in a later pregnancy placenta previa OR 1.74, about 70% higher; placenta accreta OR 2.95, about 3 times; placental abruption OR 1.38; stillbirth OR 1.27. That directly affects whether you want a second child. On the other side, people who had a cesarean later have less urine leakage and pelvic organ prolapse. These all come from follow-up record studies; the authors say you cannot conclude from them that the cesarean caused the difference

27.11 Before pregnancy, confirm your maternity insurance enrollment; an unemployed spouse can also claim maternity medical expenses

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence A

For Pregnant or planning

In plain terms

When the employer pays maternity insurance, medical costs of childbirth and the allowance during maternity leave both come from that fund. If the spouse has no job, the medical-cost part can still be claimed. Whether you have enough months of enrollment, and whether birth outside the local area needs advance filing, must be asked early.

Cost

No money spent. One call to the social insurance or yibao (医保 — basic medical insurance) advice line

Benefit

Article 54 of the Social Insurance Law: when the employer has paid maternity insurance premiums, the employee enjoys maternity insurance benefits. An unemployed spouse of the employee enjoys maternity medical expense benefits under national rules, paid from the maternity insurance fund. When the unit pays maternity insurance, the employee gets benefits, and medical costs when that unemployed spouse gives birth also come from the same fund. Maternity insurance benefits have two parts: maternity medical expenses — the medical costs of childbirth itself — and the maternity allowance — the money paid to you during maternity leave. Article 56 lists three situations for the allowance: female employees on maternity leave after birth, leave for family-planning procedures, and other situations set by laws and regulations. The maternity allowance is “paid according to the employer’s average monthly wage of employees in the previous year” — the unit’s prior-year average, not your own wage

Notes

Ask the local medical insurance authority two things early. First, whether continuous enrollment months are enough, and whether there is a minimum. Second, if birth is not in the enrollment area, whether you must file an out-of-area maternity record first, and how costs are claimed without filing. Rules differ by place; this text does not check each locality — asking early is the author’s experience. Leave days and how the allowance is calculated are in Section 18, item 2

27.12 Before discharge, get the Medical Certificate of Birth done; choose the name in advance and do not write the wrong characters

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Time
Value: HighEvidence A

For Pregnant or planning / Child under 3 at home

In plain terms

This certificate is the prerequisite for hukou (户口 — household registration), insurance enrollment, and school. If birth was in a hospital, collect it from the facility that attended the birth. If birth was outside a midwifery facility, you need a parentage test first. For characters in the name, confirm the hukou system can type them.

Cost

No money spent. Done as part of discharge paperwork

Benefit

Article 23 of the Maternal and Infant Health Care Law: medical and health-care institutions and persons engaged in home delivery issue a uniformly produced medical certificate of birth for the newborn under rules of the health administration under the State Council. This certificate is the prerequisite for everything that follows: hukou registration, insurance enrollment, a medical insurance card, and kindergarten or school all need it. The State Council General Office’s 2015 opinion on registering hukou for people without one states: for births inside a midwifery facility, apply to that facility for the Medical Certificate of Birth. For births outside a midwifery facility, first obtain a parentage test from a qualified appraisal body, then apply to the agency entrusted by the county-level health department

Notes

Before filling it in, check parents’ names, ID numbers, and the child’s name character by character. Name characters must be typeable in the hukou system; rare characters make later paperwork hard. Changing a name or the certificate means reissue or replacement, which is far more trouble than checking on the spot. Keep the certificate separately once you have it — as important as the household register and ID card

27.13 Do not refuse newborn heel-prick screening or hearing screening

  • Money Low
  • Time Minutes
  • Willpower None
  • Benefit Big
  • Measured in Lifespan
Value: HighEvidence A

For Pregnant or planning / Child under 3 at home

In plain terms

One heel-prick blood draw and one hearing test can find hypothyroidism, phenylketonuria, and hearing loss while the child still looks completely normal. By the time symptoms show, damage to intellect can no longer be undone.

Cost

Free in most areas, or a few dozen yuan. Some expanded items you pay yourself. One heel prick after the child is born

Benefit

Article 3 of the Administrative Measures for Newborn Disease Screening: “National newborn disease screening covers neonatal inherited metabolic diseases such as congenital hypothyroidism and phenylketonuria, and hearing impairment.” These conditions share one point: early on there is no visible abnormality; once symptoms are obvious, intellectual damage is already irreversible. Early detection and early intervention work well — thyroid hormone replacement, special formula milk, hearing aids, or cochlear implants. Article 10 requires that when a screen is positive, the facility promptly notify the guardian to seek confirmatory diagnosis

Notes

Article 11 requires that before screening, the items, conditions, methods, sensitivity (how many cases can be found), and fees be told to the guardian truthfully, with signed consent. So you will get an informed-consent form. Before signing, see clearly which conditions are nationally required and which are self-pay add-ons. A positive first screen is not a confirmed diagnosis; under Article 10 the facility will notify you to take the child for confirmation — do not fall apart at this step. If the first hearing screen is not passed, go for the repeat screen as notified; do not decide on your own that “the child can hear, so it is fine”

27.14 Enroll your newborn in yibao right after getting the birth certificate — don’t wait until household registration is complete

  • Money Low
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: OrdinaryEvidence A

For Pregnant or planning / Child under 3 at home

In plain terms

As soon as you receive the birth certificate, you can enroll your baby in yibao online — no need to wait for household registration. Once enrolled, the hospital bill can be reimbursed right at discharge. Newborn hospital bills can be quite high, so having yibao makes a big difference.

Cost

You pay the yibao premium just once a year; it usually costs a few hundred yuan.

Benefit

According to the Chinese government website in 2024, “Newborns can enroll in yibao online using their birth certificate; household registration is not required first.” It also states that “after discharge, newborns can get medical expenses reimbursed immediately.” Previously, families had to complete household registration first, pay all hospital costs out of pocket, and then apply for reimbursement. The average processing time for enrollment has also dropped from 28.7 working days at the start of the year to just 6.4 working days now.

Notes

Premature birth, phototherapy for jaundice, and neonatal pneumonia are common reasons newborns end up in the hospital — often resulting in bills of over 10,000 yuan. Having yibao versus not having it makes a huge difference in such cases. Local rules vary regarding how many days after birth a child must be enrolled to qualify for coverage starting from birth; always check with your local medical insurance office for exact details. Before discharge, calling 12393 is the easiest way to get clear answers.

27.15 Within one month after the child is born, go to the police station to file the birth registration

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Small
  • Measured in Freedom
Value: OrdinaryEvidence A

For Pregnant or planning / Child under 3 at home

In plain terms

Within one month after the birth you must report the birth for hukou (户口, household registration), and you must also get a vaccination record at the hospital where the child was born or at the vaccination site where you live. These are two separate matters. A child who has no hukou yet must still be vaccinated on time; do not wait until the hukou is done.

Cost

No money spent. One trip to the hukou (户口 — household registration) window at the police station

Benefit

Article 7 of the Household Registration Regulations (《户口登记条例》): "Within one month after the infant is born, the household head, a relative, a caregiver, or a neighbor shall report the birth registration to the household registration authority at the infant's place of habitual residence." This is a deadline set by law, not advice. Hukou registration and vaccination are handled separately. The Vaccine Administration Law (《疫苗管理法》) requires that within one month after the child is born, you get a vaccination record at the vaccination site where you live or at the hospital where the child was born. These sites may not refuse. Vaccination is managed by place of residence, and the child does not need to be registered first. A child with no hukou must still be vaccinated promptly on the immunization schedule

Notes

Bring these: the Medical Certificate of Birth, the parents’ ID cards and household registers, and the marriage certificate; the exact list follows what the local police station requires. The State Council General Office’s 2015 opinion states voluntary choice of father’s or mother’s hukou — you may choose either side. School districts and insurance enrollment areas may differ; decide before you go, because changing later is trouble. Children born outside marriage can also register hukou: the same opinion says you may apply with the Medical Certificate of Birth, one parent’s household register, and a statement of non-marital birth. For registration under the father’s hukou, you also need a parentage test from a qualified appraisal body

27.16 Do not skip the 42-day postpartum checkup; in some places it is also a postpartum depression screen

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence A

For Pregnant or planning

In plain terms

This checkup costs nothing. On local published service lists it includes a complete blood count, a vaginal discharge exam, and one postpartum depression screen. Serious problems can still appear in the first year after birth; do not assume you are safe once the confinement month ends.

Cost

No money spent; it is part of basic public health services. Half a day. The hard part is not wanting to move just after the confinement month

Benefit

Under maternal health management in the National Basic Public Health Service Standards (Third Edition), after birth there is 1 postpartum home visit and 1 health checkup at 42 days postpartum. On local published service lists, that visit includes lab tests (complete blood count, vaginal discharge exam) and 1 postpartum depression screen. The National Health Commission’s 2020 work plan for specialized depression prevention services asks pilot areas to “include perinatal depression screening in routine prenatal checkups and postpartum visit workflows.” So whether depression screening comes with the postpartum visit depends on whether the locality does this. Serious complications can still appear in the first year after birth. On the “go to the hospital immediately” list in Item 7, “thoughts of harming yourself or the child,” “postpartum vaginal bleeding or abnormal discharge,” and “severe swelling and pain in one limb” all belong to the postpartum period

Notes

Postpartum depression is not being oversensitive, and one family line of “cheer up” does not fix it. There are dedicated screening questionnaires and treatment, and recovery is possible. What to do with suicidal thoughts is in Section 1. Legal options when you truly cannot raise the child are in Section 9, item 20. This checkup also looks at how the wound is healing, how the uterus is recovering, and pelvic floor function. Contraception is discussed then too: no period after birth does not mean you cannot get pregnant

27.17 You do not need to rehome the cat when pregnant; have someone else scoop the litter every day, cook meat through, and wear gloves when you handle soil

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence B

For Pregnant or planning

In plain terms

Toxoplasma is a parasite. A first infection in pregnancy can cause miscarriage, and the child’s eyes and brain can be damaged. Living with a cat does not make infection more likely, so the cat does not have to go. What matters more is undercooked meat and handling soil without washing hands; having someone else scoop the litter every day is enough.

Cost

No money spent. Hand litter scooping to someone else in the household every day. If only you can scoop, buy a box of disposable gloves. The hard part is eating meat fully cooked — no rare steak or underdone lamb

Benefit

The US Centers for Disease Control and Prevention says a first toxoplasma infection shortly before or during pregnancy can pass to the fetus. Most infected children have no symptoms at birth but may lose sight or develop intellectual disability later. A few are born with severe eye or brain damage. The UK National Health Service (NHS) says infection in pregnancy can cause miscarriage. A toxoplasmosis testing centre in Lyon, France, counted 603 women with infection confirmed in pregnancy, most of whom took antiparasitic drugs. Overall, 29% of infections passed to the fetus (95% CI 25 to 33, the plausible range). About 6% of infections at 13 weeks passed on, versus about 72% at 36 weeks. The earlier the infection, the more likely the child shows symptoms. Infection at 24 to 30 weeks carried the highest risk of early symptoms in the child, about 10%. A comparison across six European cities of newly infected and uninfected pregnant women found: living with a cat OR 1.0 (95% CI 0.7 to 1.5), no difference; cleaning the litter box yourself OR 1.3 (95% CI 0.8 to 2.1), a difference that may be chance; eating undercooked lamb OR 3.13 (95% CI 1.4 to 7.2), about 3 times the risk of not eating it; contact with soil OR 1.81 (95% CI 1.2 to 2.7), about 80% higher. Across the cities, 30% to 63% of infections could be tied to undercooked or cured meat, and 6% to 17% to soil. CDC advice for pregnant people: let someone else change the litter if possible; if no one can, wear disposable gloves and wash hands with soap and water afterward. Change litter daily, because the parasite in cat feces takes 1 to 5 days to become infectious to people. Do not get a new cat in pregnancy, and do not adopt or handle stray cats, especially kittens. Keep the cat indoors and feed only cat food or cooked food, never raw meat. Explainers from the Chinese CDC also say the father-to-be should take over the litter box when possible, and that with precautions in pregnancy the cat can stay. The NHS states that stroking or owning a cat does not give you toxoplasmosis

Notes

A cat usually sheds toxoplasma in feces only for a few weeks after its first infection. An adult cat infected before is even less likely to shed again. A cat kept indoors and not fed raw meat has little chance of getting infected. You can catch toxoplasmosis without owning a cat; sources are undercooked meat, unwashed fruit and vegetables, soil contaminated by cat feces, and untreated water. So do these: cook meat all the way through, and wash knives, cutting boards, and hands after raw meat; rinse fruit and vegetables under running water; wear gloves for gardening or turning soil, and wash hands afterward. The NHS also advises against cured raw meats such as salami and Parma ham, and against unpasteurised goat’s milk. The European study did not ask in detail about lettuce and unwashed vegetables, and 14% to 49% of infections could not be traced. Ask your prenatal doctor whether to test for toxoplasma antibodies. The grade is B because the cat and meat risks rest on one study’s figures, recalled after the fact. The practices come from official bodies; no study has calculated how many infections following them would prevent. The “medium” benefit rating is a judgement call, with the reasoning in the verification record. The benefit falls on you and your child

27.18 If flu season falls during pregnancy, get one shot of inactivated influenza vaccine; any stage of pregnancy is fine; do not use the nasal spray

  • Money Low
  • Time Minutes
  • Willpower None
  • Benefit Big
  • Measured in Lifespan
Value: HighEvidence ADisputed

For Pregnant or planning

In plain terms

Pregnant people who get flu are more likely to be hospitalized than people who are not pregnant. One flu shot in pregnancy cuts your own flu hospitalizations by about four tenths. In the child’s first 6 months, confirmed flu falls by about half, and flu hospitalizations by about seven tenths. No rise in miscarriage, stillbirth, or birth defects was seen after the shot.

Cost

Self-pay, a few dozen to a little over 100 yuan per shot. One trip to a community vaccination clinic. The hard part is picking a product whose package insert does not list pregnancy as a contraindication

Benefit

The WHO 2022 position paper pooled existing studies. Pregnant people with flu had about 7 times the hospitalization risk of people who were not pregnant (OR 6.8, 95% CI 6–7.7, the plausible range). A trial in Bangladesh randomized 340 pregnant women into two groups. One got inactivated influenza vaccine; the other got pneumococcal vaccine as a control, followed to 24 weeks after the child was born. In the flu-vaccine group there were 6 confirmed flu cases in children versus 16 in controls, vaccine efficacy 63% (95% CI 5–85). Mothers’ own fever plus respiratory illness fell 36% (95% CI 4–57). Two South African trials used placebo; neither doctors nor pregnant women knew which shot was which. Among pregnant women without HIV, confirmed flu fell from 3.6% to 1.8%, efficacy 50.4% (95% CI 14.5–71.2). Their children fell from 3.6% to 1.9%, efficacy 48.8% (95% CI 11.6–70.4). Pooling 4 trials and 3 observational studies, confirmed flu in children under 6 months was about 48% lower (95% CI 33–59). Four observational studies looking at child hospitalization for confirmed flu found about 72% lower (95% CI 39–87). Another pool of only two trials found confirmed flu 34% lower (95% CI 15–50). But children’s “influenza-like illness” did not differ — that is illness that looks like flu without lab confirmation. Pregnant people’s own hospitalizations were read from records in four countries from 2010 to 2016. Hospitalization for confirmed flu was about 40% lower among those vaccinated (95% CI 12–59). On safety, one pool took in 40 studies. Stillbirth after vaccination aOR 0.84 (0.65–1.08), congenital malformation 1.03 (0.99–1.07). Both differences sat within error. Preterm birth 0.87 (0.78–0.96), about 13% lower. The US checked three flu seasons from 2012 to 2015. Vaccination in the 28 days before miscarriage aOR 0.9 (0.6–1.5), no association. The Chinese CDC’s 2026–2027 season guideline lists pregnant people as a priority group. It says vaccination at any stage of pregnancy can protect infants under 6 months of age, with a better effect in late pregnancy. Live attenuated vaccine is forbidden in pregnancy

Notes

Disputed: A 2017 US study found more miscarriages among people vaccinated in the 28 days before miscarriage (aOR 2.0, 1.1–3.6). The excess appeared only in those who had also received an H1N1-containing vaccine the prior season. The authors themselves said this does not prove causation. The same group later checked three more flu seasons and did not see the link again, so countries still say any stage of pregnancy is fine. Some domestic flu vaccine inserts still list pregnancy as a contraindication. Guidelines advise picking a product whose insert does not list pregnancy as a contraindication, and telling the vaccinator you are pregnant. Use only injectable inactivated vaccine. The nasal spray is live attenuated vaccine; domestically it is only for ages 3 to 17, and pregnant people must not receive it. Best to finish before the local flu season starts, usually September to October. If you miss that window, you can still catch up through the whole flu season. Children under 6 months cannot get flu vaccine, so this pregnancy shot is the main protection for the child’s first months. People who live with you and who care for the child are also a priority group; the adult account of flu vaccination is in Section 1, item 20 (get a flu vaccine). The Benefit line has no mortality figures; the benefit magnitude is set by confirmed flu and hospitalization, with the reasoning in the verification record. The benefit falls on you and your child

27.19 From 28 weeks of pregnancy onward, fall asleep on your side — left or right is fine; do not fall asleep on your back

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Big
  • Measured in Lifespan
Value: HighEvidence BDisputed

For Pregnant or planning

In plain terms

Falling asleep on your back after 28 weeks raises the risk the baby dies in the womb to about 2.6 times that of side sleeping. Left and right are the same; pick the side that is comfortable. If you wake at night and find yourself on your back, do not panic — roll to the side and sleep on. That figure comes from asking people afterward, and there is controversy.

Cost

No money spent. The hard part is changing posture if you are used to sleeping on your back

Benefit

Stillbirth here means the baby dying in the womb after 28 weeks. One pool merged raw data from five studies. Four were in New Zealand, Australia, and the UK; one was an online international survey. There were 851 pregnant women with stillbirth and 2257 with a live child as controls. The method was to ask afterward how they had fallen asleep most recently. Versus falling asleep on the left, falling asleep on the back had stillbirth risk aOR 2.63 (95% CI 1.72–4.04, the plausible range), about 2.6 times. Falling asleep on the right aOR 1.04 (0.83–1.31), no difference from the left. The association was similar when the child was small, the pregnant person was obese, or smoked. The authors estimated that if all pregnant people after 28 weeks fell asleep on their side, stillbirths in that stage would fall by about 5.8% (3.2%–9.2%). The UK National Health Service (NHS) lists “do not fall asleep on your back after 28 weeks” among ways to lower stillbirth risk. It also says that waking up on your back is not a reason to worry — roll to the side and sleep on

Notes

Disputed: The figures above all come from asking afterward. People whose baby died may remember inaccurately; the authors acknowledge that. There is also no randomized trial. A US study asked 8706 first-time pregnant women about sleep position in advance, then looked at outcomes. Through 30 weeks, back sleeping or not sleeping on the left had no link to stillbirth, a small baby, or pregnancy hypertension (aOR 0.99, 0.89–1.11). But it only reached 30 weeks; sleep position in the later weeks was not recorded. Side sleeping costs nothing and has no downside, so the UK still advises it. Stillbirth itself is uncommon; in high-income countries late stillbirth is about 1.3 to 8.8 per 1000 births. The “large” benefit rating follows the 2.6-times figure mechanically, with the reasoning in the verification record. The benefit falls on the child

For Pregnant or planning

In plain terms

You can conceive again as early as 3 weeks after birth, even while breastfeeding and before periods return. Breastfeeding only blocks pregnancy when all three hold: exclusive breastfeeding, no period yet, and the child under 6 months. Conceiving again within half a year after birth raises the next child’s preterm risk by about four tenths. The World Health Organization advises waiting at least two years after birth before trying again.

Cost

A box of condoms is a few dozen yuan. An IUD or an implant means a hospital visit; prices vary by place. The hard part is postpartum chaos leaving no room for this

Benefit

The UK National Health Service (NHS) says you can conceive again as early as 3 weeks after birth, even while breastfeeding and before periods return. Unless you want another pregnancy right away, use contraception every time you have sex. Relying on breastfeeding is the lactational amenorrhea method; it needs all three at once: breast milk only, no period yet, and the child under 6 months. Done by the rules, it blocks over 99%. When that standard is not met, about 2 in every 100 people conceive within 6 months. Switch to another method if any of these appear: the child passes 6 months, formula or complementary food or a pacifier is introduced, a period returns (even light bleeding counts), night feeds stop, or feed frequency falls. One pool merged 67 studies and adjusted for maternal age and household circumstances. Using 18 to 23 months from prior birth to next conception as the reference, intervals under 6 months had preterm OR 1.40 (95% CI 1.24–1.58, the plausible range), about 40% higher; low birth weight 1.61 (1.39–1.86); child small for gestational age 1.26 (1.18–1.33). Intervals of 6 to 17 months and over 59 months were also higher on these three. A Canadian study of more than 148000 pregnancies compared a 6-month interval with an 18-month interval. For mothers 35 and older, death or serious illness was 0.62% versus 0.26%, aRR 2.39 (2.03–2.80), about 2.4 times. For mothers 20 to 34 there was no difference (0.92, 0.83–1.02). For children of mothers 20 to 34, combined stillbirth, death, extreme smallness, or extreme prematurity was 2.0% versus 1.4%, aRR 1.42 (1.36–1.47). A 2005 WHO expert meeting advised waiting at least 24 months after a live birth before trying the next pregnancy. Experts agreed that conception under 18 months should be avoided. After miscarriage or induced abortion, wait at least 6 months

Notes

Disputed: How long is enough differs by voice. One WHO expert camp wants 18 months, another 27 months; 24 months is the compromise. The NHS says at least 12 months. An Australian study compared two intervals within the same 40441 mothers and got a different picture. For intervals of 0 to 5 months, preterm OR 1.07 (0.86–1.34), low birth weight 1.03 (0.79–1.34), both showing no clear difference. Comparing across different mothers, preterm was 1.41. So part of the short-interval risk may come from the mother’s own circumstances. Contraception itself is not in dispute; the dispute is only how long to wait. Per the NHS, condoms and implants can be used right after birth, including while breastfeeding. Combined hormonal pills for breastfeeding people usually wait until at least 6 weeks postpartum. Which method to use, discuss with the doctor at the 42-day postpartum checkup; see Item 16 (the 42-day postpartum checkup). The “medium” benefit rating is a judgement call, with the reasoning in the verification record. The benefit falls on you and the next child
