24 Seeking medical care: How to save money and avoid unnecessary hassle

Basis: money and time. This section focuses on the practical steps involved in getting medical treatment — which hospitals to visit first, how to transfer between levels of care, and where you can cut down on expenses. It also explains how to wait in emergency queues, what evaluations and documents are required after serious injuries, and what benefits you may be eligible for (see Section 7). Long-term management of chronic diseases is covered in Section 16, while immediate actions at the scene of an emergency are described in Section 13.

The content is largely based on the “Several Measures on Accelerating the Development of a Hierarchical Medical System” issued by the General Office of the State Council in April 2026. This system directs patients to hospitals according to the severity of their condition. The document explicitly links using primary-level facilities with lower medical costs. However, implementation varies by region, and what is written here is the national rule. For your own pooling area, what your local medical insurance and health authorities say prevails. Your “coordinated region” is simply the area where you’re enrolled in medical insurance — usually a city.

Items in this section (15)
  1. 24.1 For a common illness, start at the community clinic, go up by stepwise referral, and let the inpatient deductible carry forward
  2. 24.2 In the same pooling area (where you are enrolled), a lower hospital level reimburses a higher share, a gap of about 10 percentage points
  3. 24.3 To reach a large hospital, go through a primary-care referral or the hospital's referral center, and do not go to a scalper
  4. 24.4 Before going out of province for care, ask locally first. In principle, whether it is necessary is assessed by an associate chief physician or above
  5. 24.5 After every visit, keep your own copy of the records, the test reports, and the images
  6. 24.6 If you have doubts about treatment, request to seal the medical records on the spot; both parties must be present, a list must be drawn up, and each side gets a copy
  7. 24.7 For a severe injury or illness, go straight to the emergency triage desk, and do not queue at the registration window
  8. 24.8 With no money, no papers, and no way to say who you are, emergency care still has to come first
  9. 24.9 A disability assessment waits until treatment has ended, or until the clinical result is stable
  10. 24.10 If functional impairment really remains after treatment, apply for a disability certificate at the county-level disability association in your hukou area
  11. 24.11 Thank the doctors who saved you — send thank-you letters, banners, or satisfaction ratings instead of cash gifts. The rules prohibit money and goods, not gratitude
  12. 24.12 Before an implant, ask which items the flat fee includes, prefer an institution that has pledged to join the price controls, and do not trust a low-price ad
  13. 24.13 If the hospital says a nationally negotiated yibao drug is out of stock or not stocked, have the doctor write a prescription and buy it at a dual-channel designated pharmacy. It is still reimbursed under the rules
  14. 24.14 For a chronic illness already diagnosed at a hospital and now stable, a refill can be a follow-up on that hospital's internet hospital. The first visit for this illness cannot be online
  15. 24.15 During a hospital stay, ask for the bill and check it line by line. Ask about items that yibao (basic medical insurance) will not reimburse before you agree. If it does not match, complain to the hospital's price line first, and if that fails, report it to the yibao department

24.1 For a common illness, start at the community clinic, go up by stepwise referral, and let the inpatient deductible carry forward

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Money
Value: OrdinaryEvidence A
In plain terms

Go to the community clinic first. If the clinic then refers you to a large hospital for a stay, the stretch you pay at the start carries forward, and you do not pay that stretch a second time. How the arithmetic works is set locally. In Sichuan you only make up the difference between the hospitals' deductibles. If a large hospital transfers you back to the community clinic to continue the stay, the same illness is not calculated again during that period.

Cost

No money. You do have to make a trip to the community clinic first. The hard part is holding back from booking a specialist directly.

Benefit

For an insured patient referred up step by step from primary care, the inpatient deductible at the higher hospital can be calculated continuously. The deductible is the stretch you pay yourself at the start of each hospital stay. Yibao (basic medical insurance) starts reimbursing only after that stretch is paid. The State Council document does not say how continuous calculation works in detail. Localities set that. Sichuan's method is to charge only the difference between the hospitals' deductibles. The county traditional Chinese medicine hospital has a deductible of 600 yuan. Transfer to the county people's hospital (deductible 800 yuan), and you only need to pay another 200 yuan. For an inpatient transferred down from a higher hospital back to primary care, no separate deductible is set again for the same illness in the same treatment period.

Notes

Referral rules are set separately by each province, each city, and each medical consortium. Ask the community clinic before you go. For a chronic illness that is stable, do not go back to a large hospital just to refill medicine, because ordinary outpatient visits of this kind at large hospitals will be cut back over time. A community clinic can prescribe 12 weeks of medicine at a time. See Section 16, item 5 (a community clinic can prescribe up to 12 weeks of medicine at a time).

  • Money No cost

Value: OrdinaryEvidence A
In plain terms

The document requires that, for the same inpatient bill, a lower hospital level reimburses a higher share. Localities have to open a gap between levels, about 10 percentage points for each step up. On a bill of 10,000 yuan, if the share paid back drops from 80% to 70%, what you pay yourself goes from 2000 yuan to 3000 yuan. Newly added yibao (basic medical insurance) funds also tilt toward primary care.

Cost

No money. The hard part is holding back from running to a large hospital.

Benefit

The document requires localities to open a gap in inpatient reimbursement across hospital levels, in principle about 10 percentage points at each step. On that gap, each step up to a higher hospital means you pay about 10% more of the reimbursable bill yourself. Take 10,000 yuan of reimbursable inpatient charges. If the reimbursement share drops from 80% to 70%, what you pay yourself goes from 2000 yuan to 3000 yuan. The extra 1000 yuan is a tenth of the total bill, but what you pay yourself is half again what you used to pay. Actual shares differ by place, and the deductible and the cap also move the gap. This is only an illustration using the document's 10 percentage points. Newly added yibao funds for the year also tilt toward primary care.

Notes

For the same inpatient bill, the difference you pay yourself between primary care and a tertiary hospital can be a few thousand yuan. An illness that can be finished at primary care should not be taken up the ladder. That does not mean every illness has to be seen at the community clinic.

Value: OrdinaryEvidence A
In plain terms

The lead hospital in a medical consortium has to hold back a share of appointment slots and beds for people referred up from primary care. The hospital also has to set up a referral center, or name a fixed department to handle referrals, with full coverage by 2027. Failing to get a slot does not mean there is no way in. You do not need to go to a scalper.

Cost

No money. There is an extra referral step to complete.

Benefit

The lead hospital of a tight medical consortium must reserve a share of appointment slots and beds for primary care, and must take in, without delay, patients referred up from primary care. A tight medical consortium is a group of hospitals and community health institutions bound together to work as a set. A referral up from the community clinic therefore has its own slots and beds, and does not have to compete with people grabbing slots in the lobby. Medical institutions must set up a referral center or name a fixed department, with full coverage by 2027.

Notes

The reserved slots are kept for referred patients. If you cannot grab a slot, you can still go through referral. If you cannot find the referral center, call the hospital's service line and ask.

24.4 Before going out of province for care, ask locally first. In principle, whether it is necessary is assessed by an associate chief physician or above

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence A
In plain terms

Whether you need to go elsewhere for care is, in principle, assessed by a doctor at associate chief physician rank or above, at a secondary or tertiary hospital. How much temporary care away from home is reimbursed differs from care at a hospital of the same level where you are enrolled. The local rules say by how much. Asking locally before you set out is better than arriving elsewhere and only then finding it will not be reimbursed.

Cost

No money. Ask once more before you set out.

Benefit

Whether cross-pool and cross-province care elsewhere is necessary is, in principle, assessed by staff at associate chief physician rank or above at a secondary or tertiary hospital. Crossing pooling areas means leaving the place where you are enrolled in yibao (basic medical insurance). Whether it is necessary to go elsewhere is, in principle, something a doctor of sufficient rank has to say. The reimbursement level for people who temporarily seek care out of province keeps a reasonable gap from institutions of the same level where they are enrolled. For the same illness seen elsewhere, the money that comes back is a step lower than at home.

Notes

Temporary care away from home and long-term residence elsewhere are different filings, and the reimbursement level is not the same. How to file, and which illnesses can be settled directly, is in Section 16. Do not rush off elsewhere and only then think about reimbursement.

24.5 After every visit, keep your own copy of the records, the test reports, and the images

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Money
Value: OrdinaryEvidence A
In plain terms

Looking up and copying your own records is a right the law gives you. Outpatient records, the inpatient record, lab slips, images, surgery and anesthesia records, pathology material, and the bill can all be copied. The hospital has to name a department or a person to take the request, and it may charge only the materials fee. Changing hospitals, having another doctor look again, claiming on commercial insurance, and a real dispute all rest on this stack of papers.

Cost

No money. A hospital may charge a materials fee for copies, as the rules allow, and the fee schedule has to be public. The hard part is remembering to copy a set after every visit.

Benefit

The rules list looking up and copying the medical record as a patient's right. The scope covers the outpatient record, the inpatient record, lab slips, medical images, surgery and anesthesia records, pathology material, medical charges, and the rest of the file. Changing hospitals, having a second doctor look again, claiming on commercial insurance, and a real dispute all rest on these papers.

Notes

Copying is handled by the department the hospital names. You do not have to ask a doctor for a favor. For the list of what to copy, read out Article 16 of the Regulations on the Prevention and Handling of Medical Disputes. First come the outpatient record, the inpatient record, the temperature chart, the order sheet, lab slips, and images. Then come the surgery and anesthesia records, pathology material, nursing notes, and the medical bill. After you have them, file them in time order. What matters most in the folder is the discharge summary, the operative note, the pathology report, and the key images. Write each round of chronic-illness readings in the same notebook. See Section 16.

24.6 If you have doubts about treatment, request to seal the medical records on the spot; both parties must be present, a list must be drawn up, and each side gets a copy

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Money
Value: OrdinaryEvidence A
In plain terms

If you’re unsure about any aspect of your treatment, you have the right to request that your medical records be sealed. Both you and the medical staff must be present for this process, which involves creating a detailed list, signing and stamping it, and each side receiving a copy. If the records aren’t fully completed yet, only the finished portions should be sealed first; the rest can be sealed later. Should you suspect that an intravenous infusion, blood transfusion, injection, or medication caused a problem, the actual fluid bag, medicine vial, and infusion set must also be sealed together with the records.

Cost

No cost at all. Simply ask the hospital to seal the records; both you and the medical staff must be present to complete the paperwork. The real challenge is mustering the courage to say this out loud in front of the doctors.

Benefit

Regulations clearly state that “whenever a medical dispute arises and it is necessary to seal or unseal medical records, this must be done while both the patient and medical staff are present.” The sealed documents may be either originals or copies, which the hospital then stores. If the records are still being written, only the completed sections should be sealed first; the remainder can be sealed later. The hospital must produce a sealing list, signed or stamped by both parties, with each side keeping a copy. In cases where an adverse effect is suspected to stem from an IV, blood transfusion, injection, or medication, both parties must jointly seal and later unseal any related physical items. If testing is required, both parties must jointly appoint a qualified laboratory; should they fail to agree, the local health authority at the county level where the hospital is located will make the appointment. Should a transfusion-related problem be suspected and blood needs sealing, the hospital must notify the blood bank to send representatives to the scene. Additionally, there is a strict rule: “No organization or individual may alter, forge, conceal, destroy, or seize medical records.” Once a dispute is resolved or three years pass without any further claims, the hospital may unseal the documents on its own initiative (nationwide, effective 1 October 2018).

Notes

Sealing records and making copies are two distinct actions. Copying records is something you should do proactively, as explained in item 5 (always keep your own copies of medical records and imaging). Sealing, on the other hand, serves to preserve evidence at the moment a dispute arises, preventing any later alterations or additions. If you suspect that an IV, blood transfusion, injection, or medication caused a problem, you must seal not only the records but also the fluid bag, medicine vial, and infusion set; otherwise they might be removed or discarded. Hospitals are required to prominently display information about dispute resolution procedures, contact details, and to maintain a dedicated complaints department. In case of any issue, approach that department rather than confronting the attending physician directly.

24.7 For a severe injury or illness, go straight to the emergency triage desk, and do not queue at the registration window

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Time
Value: HighEvidence A
In plain terms

The emergency department does not go by who arrived earlier. It sorts people into Level I through Level IV by how serious the condition is. The place that sets the level is the triage desk at the emergency entrance, not the registration window. When you arrive, report the most dangerous thing: heavy bleeding, not being able to get air, not being fully conscious, chest pain like a heart problem, or pain you cannot stand.

Cost

No money. Walk a few more steps inside the door to find the triage desk. Say the most dangerous symptom in a single sentence.

Benefit

The National Health Commission's 2024 edition of the emergency-medicine quality-control indicators says emergency triage must "sort patients into four levels by how critical the condition is." "Level I is a critical emergency and needs treatment at once." "Level II is a severe emergency, and assessment and treatment often go on together." "Level III is an urgent case and needs to be seen within a short time." "Level IV is a non-urgent or less urgent case and is seen in order in the ordinary treatment area." Triage means that, on entering the emergency department, someone first rates how serious it is and sets the order of care. Beijing's implementation standard gives the response required at each level. Level 1: "assess and treat at once, and move the patient into the resuscitation room or the rescue room." Level 2: "monitor vital signs at once, start treatment within 10 minutes, and move the patient into the rescue room." Level 3: "treat ahead of level 4 patients, and have the patient wait in the ordinary treatment area; if the wait is longer than 30 minutes, assess again." Level 4: "seen in order, and the wait is long unless the condition changes; if the wait is longer than 4 hours, another assessment is allowed." In the Beijing standard, the manual rating indicators for level 1 include "acute disturbance of consciousness." Those for level 2 include "active or severe blood loss," "severe difficulty breathing," "chest pain that looks like a heart cause," and "severe pain from any cause (7 to 10 points)." Triage must "keep to the rule that critical and severe cases are seen first," and the entrance of the emergency department should have a triage area in a prominent place (the level standard is the national rule, and the response times are the Beijing standard).

Notes

Sorting into levels is the same across the country. How soon each level is handled is set by each province and city. This Beijing text is a version that can be checked word for word. The rating looks at heart rate, blood pressure, blood oxygen, and symptoms. Pain is among the items. Pain at 7 to 10 points can be rated level 2. The hard indicators for level 1 include the following. A heart rate above 180 beats a minute, or below 40 beats a minute. A systolic pressure below 70 mmHg. Systolic pressure is the higher reading in a blood-pressure pair. Blood oxygen below 80%. Shock, a definite myocardial infarction, or acute disturbance of consciousness. If you think triage set you too low, add the most dangerous symptom and say it clearly. If the condition changes while you wait, go back to the triage desk and get rated again at once. When it is truly critical, call 120 (see Section 13). Beijing's plan requires the pre-hospital emergency doctor to give a first rating by the standard, and for levels 1 and 2 to notify the emergency department of the receiving hospital in time so it can prepare to take the patient.

24.8 With no money, no papers, and no way to say who you are, emergency care still has to come first

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence A
In plain terms

The state has a disease emergency-assistance fund that pays emergency fees for both of these groups: people whose identity cannot be established, and people who cannot pay. A hospital must not refuse or delay the rescue, and an emergency center must not keep the ambulance in because of money. So do not stay home and endure it because you have no money, and do not skip calling an ambulance for fear of the bill. This money covers only the emergency. The hospital stay after that still goes through yibao (basic medical insurance) and medical assistance.

Cost

No money.

Benefit

The people the disease emergency-assistance system covers are "patients who suffer an acute, severe, or critical injury or illness inside China, who need emergency care, but whose identity is unclear or who cannot pay the corresponding fees." The fund pays both kinds of fees: "emergency fees incurred by a patient whose identity cannot be established," and "emergency fees left unpaid by a patient whose identity is clear but who cannot pay." The document also says "medical institutions of every kind, and their staff, must treat a patient with an acute, severe, or critical injury in time and effectively, and must not refuse, shift, or delay treatment for any reason." Pre-hospital emergency care has a rule too. The national pre-hospital medical emergency number is "120." An emergency center (station) "must not refuse, shift, or delay pre-hospital medical emergency service because of dispatch," and also "must not refuse or delay pre-hospital medical emergency service because of fees." Transport follows "the principles of nearness, urgency, meeting the professional need, and taking the patient's wishes into account" (nationwide, since 2013).

Notes

The fund pays only the "emergency fee" stretch. Treatment comes first, and the institution applies to the fund afterward. You do not collect the money at a window. If you have yibao, you still use yibao first, as usual. The application steps and the caps differ by province. In the transport principles, "nearness and urgency" come before "the patient's wishes," so the ambulance will not necessarily take you to the hospital you name. When the case is critical, the nearest place that can handle it is the rule. If what you still pay yourself afterward is still too heavy, use medical assistance. See Section 7, item 10 (medical assistance).

24.9 A disability assessment waits until treatment has ended, or until the clinical result is stable

  • Money Low
  • Time Hours
  • Willpower Some
  • Benefit Big
  • Measured in Money
Value: OrdinaryEvidence A
In plain terms

How much is paid depends mainly on which grade is given, and the grade depends on how much function is still lost after treatment. The standard says the assessment is done after treatment has ended, or after the clinical result is stable. Going for a rating before the bone has healed does not meet that requirement. An ordinary fracture that has healed and does not affect function generally does not get a grade.

Cost

The assessment fee runs from a few hundred yuan to 2000 yuan. The side that applies usually advances it. You wait until treatment has ended, or until the clinical result is stable. How long that takes depends on the injury. The doctor decides. The hard part is wanting the rating fixed before the injury has properly healed, and holding that back.

Benefit

The assessment principle in the Classification of the Degree of Disability Caused by Human Injury is that it "shall take the outcome or the end result of treatment of the injury as the basis, and shall objectively rate the loss of tissue or organ and/or the degree of functional impairment." The standard says the timing "shall be after treatment of the primary injury, and of complications that are truly linked to it, has ended, or after the clinical result is stable." The standard "divides the degree of disability caused by human injury into 10 grades, from the first grade (a human disability rate of 100%) to the tenth grade (a human disability rate of 10%), with each grade 10% apart in disability rate." The basis of judgment is "destruction of the structure of human tissues and organs, functional impairment, and how far the person depends on medical care and nursing, with due regard to the effect of the disability on social contact and on psychological factors." Clauses in the tenth grade that relate to fractures include the following. The first is "a compression fracture of a single vertebral body (compression reaching 1/3) or a comminuted fracture." The second is "fractures, or a comminuted fracture, at more than a single site on the pelvis, healed with deformity." The third is "loss of 25% or more of function in any large joint of the limbs (excluding the ankle)." Disability of the same site and the same nature must not be counted twice. Where there is disability at more than a single site, each grade should be written separately (nationwide, in force from January 1, 2017).

Notes

The three rating systems do not substitute for each other. Do not mix them up. For an assault, a traffic accident, or other personal injury, the judicial assessment uses the Classification of the Degree of Disability Caused by Human Injury, and it is used to calculate disability compensation (see Section 9). A work injury goes through a work-capacity assessment, which uses another set of ten disability grades and governs work-injury benefits (see Section 19). To receive the policy benefits for disabled people, you apply separately for a disability certificate (see item 10, the disability certificate). Before the assessment, have the records, the operative notes, and the follow-up images ready (see item 5, keep your own copy of the records and images). The standard does not say whether an early assessment comes out low or high. That is inferred from the principle "take the outcome or the end result of treatment of the injury as the basis." An early assessment rates how things look then. Function that has not yet recovered may later get better, or it may get worse. When this injury is mixed with an old problem that was already there, the opinion also has to say how large a share of the disability result this injury accounts for. That line directly affects how much is paid.

A disability certificate is the threshold for receiving disability benefits. It is not proof of an injury. Disabilities fall into 7 categories: vision, hearing, speech, physical, intellectual, mental, and multiple. Each category runs from level 1 to level 4. The process is application, acceptance, assessment at a designated institution, public posting, review and approval, and issuance. It is valid for 10 years, and when it expires it is replaced free of charge. If you disagree with the assessment result, you can apply for a new assessment.

There is no fee for issuing the certificate itself. In principle, applicants must cover the assessment and photography costs, though those in financial hardship may request exemptions. You’ll need to bring your ID, hukou registration, and three two-inch photos with a white background to the county-level disability association where your hukou is registered. Once the assessment passes, the decision is posted in your village or community for five working days, after which the association has ten working days to finalize the process. The main difficulty lies in making multiple trips and waiting through this period.

Benefit

The disability certificate is “the legal proof of a person’s disability status, disability category, and disability level, serving as a key basis for accessing national and local preferential policies.” Assessment follows the national standard GB/T 26341-2010. Applications are voluntary and administered locally. All seven disability types qualify for levels one through four. County-level associations handle applications and issuance based on evaluations conducted by provincially designated hospitals or specialized institutions. Approved cases are posted publicly in the village or community for five working days, and the county-level association completes its review “within ten working days.” No fee is charged for the certificate; assessment and photography costs are normally the applicant’s responsibility, though where conditions allow the local government may subsidize them, and for applicants in special hardship the relevant departments should be coordinated to reduce or waive them. The certificate lasts ten years and can be renewed at no cost; re-evaluation is possible if conditions change. If you disagree with the assessment result, you may apply for a new assessment at the prefecture-level disability association within ten working days. If you still disagree, the provincial disability assessment expert committee makes the final conclusion (effective nationwide since 1 January 2018).

Notes

Calculate the costs and benefits before deciding to apply. The certificate itself does not provide direct financial aid; rather, it unlocks two main subsidies: living allowances for disabled individuals in dibao households, and care subsidies for those at levels one and two requiring long-term assistance — details appear in Section 7, item 8 (the two subsidies). It also qualifies holders for local rehabilitation services, assistive devices, tax reductions, employment support, and transport perks; exact criteria vary widely by province, so check your local disability association website first, then decide whether to make the trip. This certificate, the judicial disability grade, and the work-injury work-capacity assessment are three separate things, and none replaces another — see item 9 (disability evaluation waits until treatment has ended). The certificate is for your own use only and must not be lent. If your disability no longer meets the standard, it is canceled, and you may not reapply for one year after cancellation. For multiple disabilities, the most severe category determines the classification level.

24.11 Thank the doctors who saved you — send thank-you letters, banners, or satisfaction ratings instead of cash gifts. The rules prohibit money and goods, not gratitude

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence A
In plain terms

Paying money in advance will not buy you a better place in line. Appointment slots, beds, scarce drugs, and the scheduling of surgery must not be traded for a favor. Afterward you may thank the doctor who saved your life, but do not give cigarettes, alcohol, or a red envelope of cash. If the doctor accepts them, that breaks the rules. To say thanks, fill in a satisfaction rating with care, or write a thank-you letter that names the person, or present a banner.

Cost

It costs nothing at all. Writing a thank-you letter or filling out a satisfaction survey takes about a dozen minutes.

Benefit

Item 7 of the "Nine Guidelines on Integrity in Practice for Medical Institution Staff" (Guo Wei Yi Fa [2021] No. 41): "Maintain the order of care and do not undermine fairness in seeking care. Hold to the principle of equality and build a fair care environment together. It is strictly prohibited to use medical resources such as appointment slots, beds, and scarce drugs and consumables, or care arrangements such as tests and surgeries, to accept benefits or to profit privately at public expense." Item 8: "Build a harmonious relationship together, and do not accept 'red envelopes' from patients. Keep to medical ethics and discipline yourself strictly." The same item also says "it is strictly prohibited to ask for or accept from patients and their relatives and friends gifts, gift money, consumer cards, securities, equity, other financial products, and other money or goods." Meals and entertainment paid for by the patient side are the same: "it is strictly prohibited to attend banquets, or travel, fitness, entertainment, and similar activities, that they arrange, organize, or pay for." The document also requires medical institutions to carry out their main responsibility, carry out supervision, and "resolutely investigate violations." Satisfaction rating is written into the performance assessment of tertiary public hospitals. The basis is the State Council General Office's Opinion on Strengthening Performance Assessment of Tertiary Public Hospitals (Guo Ban Fa [2019] No. 4). The document says: "The performance assessment indicator system of tertiary public hospitals consists of indicators in 4 areas: medical quality, operational efficiency, sustainable development, and satisfaction rating." It also says: "Hospital satisfaction consists of two parts, patient satisfaction and staff satisfaction..." It goes on to say the aim is to "measure patients' sense of gain and staff motivation through satisfaction ratings by outpatients, inpatients, and medical staff." This rating also has a unified national platform: "The state establishes a satisfaction management system for public hospitals..." The timetable is: "By the end of March 2019, all tertiary public hospitals nationwide are brought onto the National Health Commission's satisfaction survey platform. Localities shall use that platform and bring the survey results into the performance assessment of tertiary public hospitals." Hospitals must also handle complaints. The basis is Article 2 of the Measures for Complaint Management of Medical Institutions (National Health Commission Order No. 3, in force 2019). Complaint management means patients "putting forward opinions, suggestions, or complaint requests" on medical service conduct, medical management, medical quality and safety, and the like. The hospital is responsible for investigating, handling, and giving feedback. "Medical institutions at level two and above shall set up a doctor-patient relations office or designate a department (together, the complaint management department) to carry out complaint management in a unified way." The duties of the complaint management department include "receiving complaints in a unified way, investigating and verifying the matters complained of, proposing handling opinions, and answering patients promptly." Medical institutions "shall encourage staff to actively gather patients' opinions and suggestions on medical services, medical quality and safety, and the like" (nationwide).

Notes

Both you and your doctors benefit from this practice. It does not mean you should never express gratitude — after a night of emergency treatment, it’s natural to want to say thanks. The key is choosing appropriate methods. Cash gifts, cigarettes, and alcohol shift the risk onto the other person; thank-you letters, banners, and satisfaction ratings are fully aligned with official policies. If a doctor declines a gift you’ve already offered, it’s not personal — it’s his duty to refuse. Note also that offering gifts to doctors falls under professional ethics and hospital rules, whereas giving bribes to law enforcement or investigators constitutes a criminal offense under the law; these situations are entirely different; the latter is section 8, item 40. While thank-you letters and banners are widely accepted, there is no official rule stating they become part of personal or departmental records — this is merely anecdotal experience. Only the inclusion of satisfaction survey results in the performance assessment of tertiary public hospitals has documentary backing. Should you have doubts about treatment, request on the spot that the records be sealed (see item 6 of this section, sealing the records), and keep the records and images (item 5, keep your own copy of the records and images).

24.12 Before an implant, ask which items the flat fee includes, prefer an institution that has pledged to join the price controls, and do not trust a low-price ad

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence A
In plain terms

After the 2023 concentrated price cut for implants, the overall fee for a routine implant fell from an average of 15,000 yuan to 6000 to 7000 yuan. At a tertiary public hospital the service fee for a single routine implant is in principle no more than 4500 yuan. The implant and the crown are charged separately. Implants, dentures, and orthodontics are not reimbursed by yibao (basic medical insurance). Dental treatment items can generally be reimbursed across localities.

Cost

No money. Before the dental visit, look up the institution list once on the National Healthcare Security Administration's public account, and ask about the prices clearly in person. It takes about a dozen minutes.

Benefit

A January 2024 announcement from the National Healthcare Security Administration says the fee for a routine single-tooth implant is made of three parts. The first part is the medical service fee, charged as a flat fee. The flat fee covers "the consultation fee for the whole implant course, biochemistry tests and imaging fees, the implant placement fee, the crown placement fee, and the scanning, design, and modeling fee, and it does not include extraction, periodontal cleaning, root-canal treatment, bone grafting, or soft-tissue grafting." "In each locality, the medical service charge for a single routine implant at a tertiary public hospital is in principle no more than 4500 yuan." In economically developed regions, under the 2022 special price-control notice, it can be relaxed by at most 20%. The second part is the implant. In the national bulk purchase of January 2023, "the selected prices ran from 548 yuan to 1855 yuan, with an average a little over 900 yuan." The Sichuan healthcare security bureau, which led the purchase, reported that expensive imported brands fell from a median purchase price of 5000 yuan to about 1850 yuan, and the brands used in the largest amounts fell from about 1500 yuan to about 770 yuan. The third part is the crown, "with prices from 100 yuan to 656 yuan, and an average a little over 300 yuan." At a press conference in the second half of 2023, the National Healthcare Security Administration said local control measures were all in place by the end of April and the start of May, and that "the overall implant fee fell from an average of 15,000 yuan to about 6000 to 7000 yuan." On reimbursement: a 1999 document from the labor and social security ministry and other departments lists "dentures" (false teeth) and "non-functional cosmetic or orthopedic surgery" as not paid by basic yibao. In a 2019 reply to a political-consultative proposal, the National Healthcare Security Administration said, "Cosmetic items (including restoration and straightening of missing teeth, and similar care) are not in the scope paid by the yibao fund. Treatment items in dentistry are not excluded at the national level, and localities have generally brought them into the scope yibao pays" (nationwide, as of October 2026).

Notes

The 4500 yuan line applies only to public hospitals. Private institutions set their own prices, but the special price-control notice says they must not use a false or misleading "subsidy" or "low price" to draw patients in. A low price in an ad is often only the implant itself. Extraction, bone grafting, and the other items outside the flat fee are where the price is added. So before you sign, get a sheet that writes the price of each item. The National Healthcare Security Administration publishes, on its official WeChat account, the list of institutions that have pledged to join the price controls. Missing a tooth or a couple of teeth does not mean an implant is the only repair. A fixed denture and a removable denture are also ways to repair a gap, and their prices did not move with this cut. Ask the dentist which of them suits you. Whether an employee yibao individual account can pay for an implant has no single national answer. Ask the local yibao department. For how an individual account is used for family members, see Section 5, item 13 (family pooling). For why a missing tooth should be seen, see Section 2, item 10 (see a dentist about a missing tooth). The benefit magnitude is set at medium: the price cut is brought by policy, and what this step can save you is mainly the add-on outside the flat fee and the share charged by a high-priced institution, a few thousand yuan for a single tooth.

24.13 If the hospital says a nationally negotiated yibao drug is out of stock or not stocked, have the doctor write a prescription and buy it at a dual-channel designated pharmacy. It is still reimbursed under the rules

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Big
  • Measured in Money
Value: Very highEvidence A
In plain terms

If the hospital says a nationally negotiated drug is out of stock or not stocked, that does not mean the drug cannot be reimbursed. If the drug is in this province's dual channel, have the doctor write the prescription as usual and buy it at a dual-channel designated pharmacy. The reimbursement policy is the same as buying it at the hospital. Under the national document, the hospital also must not delay stocking it on the grounds of the yibao (basic medical insurance) total, or of the share of revenue from drugs.

Cost

No money. You need the doctor to write a prescription, and then a trip to the designated pharmacy. Some places require a drug-eligibility confirmation first.

Benefit

A negotiated drug is a drug the National Healthcare Security Administration priced with the manufacturer and then put in the yibao catalog. A 2021 policy explainer from the administration says the catalog is adjusted once a year, and the sequence became "into yibao first, and into the hospital after," so some negotiated drugs are "hard to get into the hospital." For that, the National Healthcare Security Administration and the National Health Commission issued Yi Bao Fa [2021] No. 28. The document says the dual channel is "a mechanism that supplies negotiated drugs through designated medical institutions and designated retail pharmacies" and "brings them into yibao payment at the same time." For a drug in the dual channel, "designated medical institutions and designated retail pharmacies apply a unified payment policy." Buying at a pharmacy and buying at a hospital follow the same reimbursement rules. Which drugs are included is, in principle, set by the provincial yibao department. Which pharmacies are included, the yibao department must "disclose to the public promptly and on its own initiative." The prescription is sent to the pharmacy through the prescription-transfer center in the yibao system, and the requirement is to "keep electronic prescriptions moving smoothly." In September of that year, Yi Bao Han [2021] No. 182 required that, for a negotiated drug the hospital cannot stock for the moment, the hospital "raise access to the drug through the dual channel and similar routes." Negotiated drugs used appropriately are listed separately and are not counted in the hospital's drug-share assessment. The document also says "the landing of negotiated drugs must not be affected on the grounds of a yibao total cap, a limit on how many drugs are in the institution's formulary, the drug-revenue share, or similar reasons." It also required that, before the end of November 2021, each prefecture-level city have at least 1 dual-channel retail pharmacy. The notice on the 2025 edition of the yibao drug catalog (Yi Bao Fa [2025] No. 33) repeats that sentence, "must not, on those grounds." The notice also says that, in principle, a negotiated drug must not be taken out of the dual channel just because it has been moved into the regular catalog. Designated pharmacies that dispense dual-channel drugs "must all receive this kind of prescription through the electronic prescription center." The new catalog takes effect on January 1, 2026 (nationwide, as of October 2026).

Notes

The method has four steps. First, ask the doctor whether this drug is a dual-channel drug in this province, and whether a prescription can be written. The prescription usually goes out as an electronic prescription in the yibao system. Second, find which pharmacy can sell it. Each provincial healthcare security bureau publishes the drug list and the pharmacy list, and you can also call the local yibao department and ask. The national yibao service app opened a search, in 2021, for institutions that stock negotiated drugs. Third, take your yibao card, or the electronic yibao certificate, and buy it. The pharmacy has to check that the person on the prescription and the person taking the drug are the same person. Fourth, ask the reimbursement share, whether it counts as an outpatient chronic or special disease, and whether it can be paid on its own. All of that is set locally. Not every negotiated drug is in the dual channel. A drug that is not can be bought only at the hospital. Whether a purchase elsewhere can be settled directly, the document only says "advance it prudently." Ask before you set out. For how outpatient chronic and special diseases are recognized, and for settlement away from home, see Section 16, item 2 (recognition of outpatient chronic and special diseases). The benefit magnitude is set at large, by judgment: no national document was found that says how much a single patient can be reimbursed. The policy explainer says the focus of what is brought into the dual channel is drugs that are "high in clinical value, urgent in demand, and high in cost." Over a course of treatment, the gap between paying yourself and being reimbursed is estimated at the level of 10,000 yuan. The beneficiary is mainly you and your family.

24.14 For a chronic illness already diagnosed at a hospital and now stable, a refill can be a follow-up on that hospital's internet hospital. The first visit for this illness cannot be online

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Time
Value: HighEvidence A
In plain terms

If a chronic illness was diagnosed at a hospital, the condition is stable, and the medicines to be prescribed are still those same ones, you can do a follow-up online with the original hospital, and the medicine can be mailed home. Where local yibao has brought this in, you can settle it directly with yibao. The first visit for this illness, or a change in the condition, means the doctor must send you to the hospital to be seen in person.

Cost

No money. You find the hospital's internet-hospital entrance on your phone and complete real-name verification. The consultation fee for an online follow-up at a public hospital is charged as an ordinary outpatient visit. If the medicine is mailed home, you pay the delivery fee yourself. Yibao (basic medical insurance) does not reimburse it.

Benefit

What you save is the time of each round trip to the hospital and of queuing to register. The Interim Measures for the Administration of Internet Diagnosis and Treatment say that online care can cover follow-up visits for only some common illnesses and chronic illnesses. The condition is that an in-person hospital has already made a clear diagnosis, the doctor has your records, and the follow-up is for that same diagnosis. The measures say "internet diagnosis and treatment must not be provided to a patient on a first visit." If the condition changes, or the doctor needs to examine you in person, the hospital must stop the online care at once and direct you to an in-person hospital. An online prescription needs the physician's electronic signature. After a pharmacist checks it, a third party may be entrusted to deliver the medicine to your home. Anesthesia drugs, psychotropic drugs, and other specially controlled drugs cannot be prescribed online. Prescribing online for a child 6 or younger requires a guardian and a physician in the relevant specialty to be present. Reimbursement by yibao is in the National Healthcare Security Administration's 2020 document Yi Bao Fa [2020] No. 45. It says that, for a follow-up prescription at an "internet-plus" designated hospital in the local pooling area, the consultation fee and the drug fee can be paid under the local yibao rules. The part you pay yourself can be paid, under the rules, from an employee yibao individual account. The fee for delivering the drugs is not in yibao. Localities may start with outpatient chronic and special diseases and gradually widen what is paid. The local yibao agency must publish the list of such designated hospitals. Yi Bao Fa [2019] No. 47 says that an internet follow-up at a public hospital, whichever rank of doctor sees you, is charged at the price of an ordinary outpatient consultation item (nationwide, as of October 2026).

Notes

First ask the attending doctor whether this illness can be a follow-up online, then find the internet-hospital entrance in the hospital's public account or app. Use the internet hospital opened by the hospital that diagnosed you. Whether yibao can settle it is set locally. Check the designated list published by the local yibao department, or call and ask. For someone enrolled elsewhere, the document only says "explore" direct online settlement away from home. Ask clearly before you use it. If the condition is stable and the community clinic has your drug, the clinic can prescribe 12 weeks at a time. See Section 16, item 5 (a community clinic can prescribe up to 12 weeks of medicine at a time). If the community clinic does not have this drug, or a specialist has to watch the dose and adjust it, an online follow-up fits better. The benefit magnitude is set at medium, by judgment: a trip to a large hospital once a month to refill medicine takes about half a day, which comes to about an hour a week. The beneficiary is mainly you and your family. Elders who have trouble getting around save the most.

24.15 During a hospital stay, ask for the bill and check it line by line. Ask about items that yibao (basic medical insurance) will not reimburse before you agree. If it does not match, complain to the hospital's price line first, and if that fails, report it to the yibao department

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Medium
  • Measured in Money
Value: OrdinaryEvidence A
In plain terms

During a stay you can ask for a bill. It lists the unit price, the quantity, and the date of each drug and each test. For a drug or a test that yibao will not reimburse, other than emergency rescue, the hospital must have your agreement, or a family member's, first. If you find a repeated charge, or a charge for something that was not done, call the hospital's price-complaint line first. If the hospital does not accept it, report it to the yibao department.

Cost

No money. During the stay, ask for a bill every few days and check it. A pass takes about a dozen minutes. The hard part is putting aside the embarrassment of asking the doctors and nurses.

Benefit

The Regulations on the Supervision and Administration of Medical Security Fund Use say a designated hospital must "truthfully issue fee documents and related materials to the insured person." The insured person also has the right to require the hospital to issue them truthfully. The hospital must not over-test, must not charge twice for the same item, must not charge above the standard, and must not split an item into pieces to charge for it. For a drug or a service outside the scope the yibao fund pays, other than an emergency or a rescue, the insured person, or a close relative or guardian, has to agree. If these rules are broken, the yibao department orders a correction. If the yibao fund loses money, it orders the money returned, and then a fine of 1 to 2 times the loss. Using an item yibao does not reimburse without agreement, and refusing to correct it, draws a fine of 10000 to 50,000 yuan. The National Health Commission's 2019 Provisions on the Administration of Internal Price Conduct of Medical Institutions require hospitals to set up a bill system, including an electronic bill. When a patient needs it, the hospital must print it. The bill must state the name and code of the item, the drug, and the consumable, and also the unit price, the pricing unit, the date of use, the quantity, and the amount. Care charged by disease type, or by DRG (a bundled payment grouped by disease), is not in this requirement. The hospital must post, in a prominent place, the phone numbers for price inquiries and complaints. Complaints follow a rule that the first person asked is responsible, and the person who receives the complaint must record the content and the result. A report to the yibao department goes to the website, phone, fax, or mailing address it has published. It can be anonymous. Personal information in a report filed under a real name must be kept confidential. If the report is verified, the reporter is rewarded under national rules (nationwide, as of October 2026).

Notes

A report to the yibao department is about the hospital misusing the yibao fund. What the regulations write is an order to correct, a return of yibao fund money, and a fine. They do not write that the extra you paid is returned to you. Getting back the extra you paid yourself depends mainly on the hospital correcting the charge, so go to the hospital's price-complaint window first, and keep the bill and the record of what was said. Suing in court over a few hundred yuan, with the ordinary procedure of a first-instance trial required by rule to finish within 6 months (which can be extended) and the lawyer's fee paid by you, is usually not worth it. A report has to give a concrete lead: which line on the bill, which day, and how many times it was charged. If you only want to ask about reimbursement policy, call the yibao inquiry line. That is not a report. An item you cannot read is worth asking the nurses' station or the admissions office about. It is not necessarily an improper charge. A drug yibao will not reimburse is not necessarily a drug you should not use. Ask how much it costs per day, and whether there is a reimbursable substitute, before you sign. At discharge, ask for a full bill and keep it with the records. See item 5 of this section (keep your own copy of the records and images). The benefit magnitude is set at medium, by judgment: no official figure was found for how much a single patient overpays. Under this book's thresholds, a few hundred to a few thousand yuan counts as medium. The beneficiary is mainly you and your family. Checking the bill for an elder in hospital matters especially.
