20 How to care for a newborn
Scope: infant mortality and money. This section only covers what affects whether the child survives or how much you spend, and only where the gap is large enough and the evidence is solid. Soothing and day-to-day parenting tips are not covered here. For tasks before birth and on the day of discharge, see Section 27.
Items in this section (17)
- 20.1 During infancy, put the baby to sleep on their back on a firm surface; share a room but not a bed; and do not place any soft items in the crib or bed
- 20.2 Give the 1st dose of hepatitis B vaccine within 24 hours after birth
- 20.3 Finish the vaccines on the national immunization schedule; they are free the whole way, and a missed series means catching up only the doses not yet given
- 20.4 For the first 6 months, feed only breast milk, not even water; from 6 months add complementary foods and keep breastfeeding
- 20.5 Mix formula with water above 70 °C, let it cool before feeding, and throw away what is left
- 20.6 Do not feed honey to a child under 1 year of age
- 20.7 The vitamin K shot at birth has to be given
- 20.8 If an infant under 3 months reaches 38 °C, go to the hospital directly and do not watch at home
- 20.9 No matter how tired or angry you are, never shake a baby
- 20.10 For diapers, ignore the brand and check 3 things: the fit, how often you change it, and whether a sampling notice has named it
- 20.11 For big items, consider them in the order borrow, buy used, buy new, and do not kit everything out all at 1 time
- 20.12 If the child has severe eczema or an egg allergy, do not avoid peanuts; add them early under a doctor's guidance, but never feed a whole peanut
- 20.13 Watch these newborn jaundice signals: yellow within 24 hours of birth, or cannot be woken and will not feed, go to the hospital that day; getting yellower, or pale stools, go and get it checked soon
- 20.14 If you are eating vegetarian while pregnant or breastfeeding, take vitamin B12 or eat fortified foods; the child's complementary foods need animal foods, and if you insist on a fully vegan diet, see a doctor first
- 20.15 When buying things for an infant, do not buy a wheeled baby walker; if you want a place to sit and play, use a stationary activity center with no wheels, or a play yard
- 20.16 From birth, give the child 400 international units of vitamin D every day, and especially do not skip it if the child is breastfed; a full-term baby generally does not need extra calcium in the first 6 months
- 20.17 When bathing an infant, put cold water in first and then mix in hot, and test it with the inside of the wrist or the elbow before putting the child in; if the water heater can set the outlet temperature, set it below 50 °C

While the child is still an infant, lay the child face up on a firm surface that does not incline. An inclined cradle or an infant seat cannot be used as a bed. The adult and the child sleep in the same room, but not in the same bed. Do not put soft things on the bed, such as pillows, quilts, bed bumpers, or stuffed toys. In the United States alone, about 3500 infants die each year from causes related to sleep.
- Cost
This costs nothing upfront. However, you’ll need to replace the mattress with a firm one, which may cost several hundred yuan. The real challenge is resisting pressure from older relatives to deviate from this rule.
- Benefit
Each year, about 3,500 infants in the U.S. lose their lives to sleep-related issues, including sudden infant death syndrome, deaths of unknown cause, and accidental suffocation in bedding. The American Academy of Pediatrics recommends a safe sleep environment consisting of: infants sleeping on their backs; using a firm, non-inclined sleeping surface; sharing a room without sharing a bed; and avoiding soft bedding and overheating. Additional protective measures include breastfeeding, keeping infants away from nicotine and alcohol, ensuring all vaccinations are up to date, and offering a pacifier.
- Notes
“Do not place any soft items in the bed” means absolutely no pillows, blankets, bed bumpers, stuffed toys, or shaped pillows — even nursing pillows that appear perfectly safe are prohibited. Inclined cribs and baby chairs must never be used as sleeping areas. In this section, this item makes the biggest difference, and it is also the one where you most need to hold out against pressure from older relatives.

The 1st dose of hepatitis B vaccine has to be given within 24 hours after the child is born. Among people infected in infancy or early childhood, about 95% go on to chronic hepatitis. People infected only after they are grown mostly get better on their own, so this dose is more useful the earlier it is given. If the mother's hepatitis B surface antigen is positive, the child also needs hepatitis B immune globulin within 12 hours of birth, at the same time.
- Cost
No money. The hepatitis B vaccine is on the immunization schedule (免疫规划) and is free to begin with. The child gets it at the hospital on the day of birth, so there is no extra trip.
- Benefit
The World Health Organization says: "All infants should receive the hepatitis B vaccine as soon as possible after birth (within 24 hours)." The reason is that "about 95% of infections in infancy and early childhood develop into chronic hepatitis." The great majority of people infected as adults clear it on their own (they get better without treatment). The WHO's own words are that "under 5%" of infections after adulthood become chronic. The National Health Commission (国家卫生健康委员会) protocol for interrupting mother-to-child transmission requires that a child born to a mother who is positive for hepatitis B surface antigen "receive the 1st dose of hepatitis B vaccine as early as possible within 12 hours after birth, and at the same time an injection of 100 international units of hepatitis B immune globulin."
- Notes
If the mother's hepatitis B surface antigen is positive, the child also needs 1 shot of hepatitis B immune globulin at the same time after birth. This has to be checked at prenatal care. Do not wait until after the birth to ask. The screening the mother herself needs is in Section 1.

Vaccines on the national immunization schedule are free for the whole series, and the vaccination provider cannot charge. Daycare and school both check the vaccination certificate, and a missed dose is generally found and made up. If you missed some, you only catch up the doses not yet finished. You do not start the whole series over, and you can continue even if the manufacturer changes.
- Cost
No money. The law states that "a vaccination provider may not charge any fee for giving a vaccine on the immunization schedule." You will make several trips to the vaccination clinic as the child ages. Keep the vaccination certificate (预防接种证). Do not lose it.
- Benefit
When a child enters daycare or school, the daycare or the school checks the vaccination certificate. If the child was not vaccinated on the immunization schedule as required, they report it to the vaccination provider and help press the guardian to catch up. So a missed vaccine is generally found when the certificate is checked at daycare or school entry. The schedule now in force is the 2026 edition. Human papillomavirus (HPV) vaccine is already on the national immunization schedule. The starting age for the diphtheria-tetanus-pertussis vaccine (百白破) changed from 3 months to 2 months. There are 5 doses in all, 1 dose at each of 2 months, 4 months, 6 months, 18 months, and 6 years. From March 2026, children of the right age in Tibet, Qinghai, Xinjiang, and the Xinjiang Production and Construction Corps (新疆生产建设兵团) also receive Japanese encephalitis vaccine (乙脑) as a routine vaccination. Several vaccines can be given on the same day: at this stage, national immunization-schedule vaccines can be given at the same time, following the schedule or the catch-up rules. 2 or more injected vaccines must go into different sites. "It is strictly forbidden to draw 2 or more vaccines into the same syringe and inject them." If they are not given on the same day, 2 or more injected live attenuated vaccines need an interval of not less than 28 days. There is no interval limit between inactivated vaccines, oral live attenuated vaccines, and other vaccines. The catch-up rule for a child under 18 years of age is to catch up as soon as possible: "only the doses not yet finished need to be given; there is no need to start the whole series over." The same vaccine can be continued if the manufacturer changes (nationwide, 2026 edition of the schedule).
- Notes
The most common misunderstanding is "the child is not strong, so wait." The official line is that a so-called "allergic constitution," allergies in the family, or a past allergy to a food or a drug is not a reason to skip the vaccine. Atopic dermatitis (eczema), hives, allergic rhinitis, and bronchial asthma can all be vaccinated as usual if the condition is stable. A child who is taking an allergy medicine, or using an inhaled steroid, can still be vaccinated. Physiological jaundice and breast-milk jaundice in a newborn can also be vaccinated as usual. There are only 2 true reasons not to vaccinate: an acute severe allergic reaction to the same vaccine before, or a severe allergy to a known component of the vaccine. A child with an immune deficiency, a child on chemotherapy or radiotherapy, or a child on an immunosuppressant needs a separate assessment. Explain the situation at the vaccination clinic and decide there. The first hepatitis B dose is in item 2 of this section. A non-schedule vaccine you pay for yourself is charged a vaccine fee and a vaccination service fee, and has nothing to do with the free vaccines this item is about.

Start feeding in the 1st hour after the child is born. For the first 6 months, feed only breast milk, not even water. From a full 6 months, add complementary foods, with iron-rich animal foods every day, and keep breastfeeding to 2 years or longer. If there is not enough breast milk, or breastfeeding is not possible, use formula.
- Cost
No money, and it saves the cost of formula. What it takes is time. Feeding takes a large part of every day. The hard part is keeping it up, and not stopping in the first 6 months.
- Benefit
The World Health Organization recommends "starting breastfeeding within the 1st hour after birth." For the first 6 months, "exclusive breastfeeding, meaning no other food or liquid, including water." Then "from 6 months of age, start safe and adequate complementary foods, and continue breastfeeding to 2 years or beyond." The National Health Commission service guide also promotes exclusive breastfeeding from 0 to 6 months. The same guide requires that from a full 6 months "the day's complementary foods must include iron-rich animal foods."
- Notes
If breast milk is not enough, or the body does not allow breastfeeding, use formula. There is no need to feel guilty. The gap between breast milk and formula is far smaller than the safe-sleep item. That is the author's judgment. How often to add complementary foods, from that Health Commission service guide: at 6 to 8 months, 1 to 2 times a day; at 9 to 12 months, 2 to 3 times a day. Each day's complementary foods are chosen from 7 food groups, and not fewer than 4. If the child has severe eczema or an egg allergy, whether to avoid peanuts is in item 12 of this section (do not avoid peanuts). If the mother eats vegetarian, or wants the child to eat vegetarian, see item 14 of this section (on a vegetarian diet, supplement vitamin B12 every day).

Formula is not sterile. It can carry a bacterium that kills 4 in 10 to 8 in 10 of the newborns who catch it. Pour the hot water in first, wait until it falls to about 73.8 °C, then add the powder. In most cases that can kill this bacterium down to below 1/100000 of what it was. Hot water leaves a bit less vitamin C, but the formulas that were measured still met the standard. Let it cool after mixing, then feed. Throw away what is left.
- Cost
No money. Each time, wait a few extra minutes for the milk to cool. The hard part is that when the child is hungry and crying, you still wait for the water to cool before you mix.
- Benefit
Formula is not sterile. It can carry Cronobacter (阪崎克罗诺杆菌). The reported death rate after a newborn is infected is 40% to 80%. Experimental work shows: pour the hot water into the bottle first, wait until the water falls to about 73.8 °C, then add the powder, and in the great majority of cases the milk is still above 70 °C. That can cut this bacterium by 5 or more orders of magnitude, down to below 1/100000 of what it was. The price of hot water shows up in vitamin C. The World Health Organization and the UN Food and Agriculture Organization held an expert meeting in 2006. The figures seen at that meeting were: when formula is mixed with boiling water, only vitamin C falls clearly, by 5.6% to 65.6% across 4 formulas. Formula leaves the factory with extra vitamin C already added. The formula that lost the most still had 9.0 milligrams per 100 kilocalories after mixing, above the Codex Alimentarius floor of 8 milligrams. The meeting saw only this 1 study, so it drew no conclusion on it.
- Notes
Disputed. The 2022 opinion of the German Federal Institute for Risk Assessment is that a healthy full-term baby can be mixed with water at 20 to 50 °C. It notes that most manufacturers also print 40 to 50 °C on the tin. Its reason is that 70 °C water cools quickly after it is in the bottle, so the kill is not as large as the calculation. Hot water also loses some nutrients, and feeding before it has cooled can burn the child. The European Society for Paediatric Gastroenterology, Hepatology and Nutrition, in 2004, also did not favor mixing with boiling water, worrying that it would damage vitamins and the probiotics added to the formula. The World Health Organization and the UN Food and Agriculture Organization still recommend above 70 °C. The highest risk is for babies born early, babies with a birth weight under 2500 grams, babies under 2 months, and babies with weak immunity. Mix those at 70 °C, or ask a doctor. For a healthy full-term baby mixed at the temperature on the tin, this German opinion considers the infection risk extremely low. The 70 °C threshold comes from public-health guidance. Beary and colleagues' own words were: "public-health guidance for caregivers of high-risk infants recommends reconstituting formula with water heated to at least 70 °C (158 °F) to inactivate microorganisms." Do not mix with water that has sat in a thermos for 1 day. Do not leave mixed milk at room temperature for the next feed.

If the child is under 1 year of age, do not feed honey. Honey water, honey mixed into complementary foods, and pastries that contain honey all count. Honey can carry the bacterium that causes botulism. The bacteria in an adult's gut can hold it down. An infant's cannot.
- Cost
No money.
- Benefit
The US Centers for Disease Control and Prevention says "do not feed honey to children under 1 year of age," because "honey may contain the bacteria that cause botulism." An adult's gut bacteria can hold down these spores (the dormant form of this bacterium). An infant's cannot.
- Notes
What must not be fed includes honey water, complementary foods mixed with honey, and pastries that contain honey.

A newborn has very little vitamin K stored. Without a supplement, bleeding can occur, and the kind that shows up late is often bleeding in the brain. In Tashkent, where this prevention was not generally done, 478 out of every 100000 newborns had this bleeding. With prevention, the estimate is that more than 78% can be cut. For a home birth, or a birth at an irregular facility, ask whether it was given.
- Cost
0 yuan to a few dozen yuan. This sum is usually already inside the hospital bill for the birth, so you do not pay it separately.
- Benefit
A newborn has very little vitamin K stored. Without a supplement, vitamin K deficiency bleeding can occur, and the kind that shows up late often presents as intracranial hemorrhage (bleeding in the brain). The Belgian consensus recommendation is "1 intramuscular injection of 1 or 2 milligrams of vitamin K at birth." Where prevention is not provided systematically, the rate of vitamin K deficiency bleeding is 478 cases per 100,000 people. That figure comes from 42000 newborns in Tashkent. The authors estimate that prevention can bring the rate down by "more than 78.5%."
- Notes
At a regular hospital in China, the hospital generally gives this shot as routine. That is the author's experience. No single nationwide official rule was found. Before discharge, look at the discharge summary, or ask. For a home birth, or a birth at an irregular facility, confirm it on your own initiative. The injection meant here goes into the muscle. That Belgian consensus calls the oral route "a slightly worse substitute," used only when the parents refuse the injection: 2 milligrams by mouth at birth, then, for a breastfed child, 1 to 2 milligrams a week for 3 months.

If the child is under 3 months and the temperature reaches 38 °C, go to the hospital directly. Do not watch at home, and do not give a fever medicine first and then wait. The American Academy of Pediatrics set a specific plan for febrile infants who still look all right, so "looking all right" is not a reason to wait at home. The UK guidance also counts a child under 3 months with a fever of 38 °C as high risk of serious illness.
- Cost
Registration plus tests, a few hundred yuan.
- Benefit
The American Academy of Pediatrics wrote a specific guideline for full-term infants from 8 to 60 days of age who still look all right. That guideline defines fever as "a temperature of 38.0 °C or above." It splits them into 3 bands, 8 to 21 days, 22 to 28 days, and 29 to 60 days, and gives each band its own path. Infants under 8 days of age are outside that guideline. The UK NICE fever guideline covers children under 5 years of age. It states that a child under 3 months with a temperature of 38 °C or above is in the "high-risk group for serious illness."
- Notes
At this age, a serious bacterial infection can show up as fever alone. So looking "all right" is not a reason to watch at home. Do not give a fever medicine yourself and then wait and see.

A baby’s head makes up a large proportion of their body size, yet their neck muscles are still very weak. Shaking them forcefully can cause bleeding inside the brain and eyes, potentially leading to death or lifelong disabilities. Gentle rocking while trying to soothe them is not dangerous; the real risk comes from uncontrolled, violent shaking. When you feel yourself losing control, place the baby back in their crib and step away for a few minutes. Letting them cry for a short while is far safer than holding them and shaking them.
- Cost
There is no monetary cost. The real difficulty lies in resisting the urge to shake a baby when you’re exhausted or upset.
- Benefit
Because a baby’s head is proportionally large and their neck muscles are underdeveloped, violent shaking can result in intracranial and retinal hemorrhage — bleeding in both the brain and eyes. This can be fatal or leave lifelong disability. Such harm is distinct from any gentle rocking used to calm them. Medically, injuries caused by shaking or impact are termed “abusive head trauma.” This condition is the leading cause of death from head injuries among children 2 years old or younger. In the United States between 1999 and 2014, it accounted for nearly 2,250 deaths of children under 5.
- Notes
When you truly feel you’re about to lose control, the safest action is to return the baby to their crib and leave the room for a few minutes. Allowing them to cry briefly is much safer than holding them and shaking them. This advice is most important for people who care for a child alone and people who have long been short of sleep. For guidance on managing your own emotions, please refer to Section 3.

No official body has ranked "which brand is best." Only 2 things can be checked: product-quality sampling notices from the State Administration for Market Regulation (市场监管总局), and regulator investigations of a specific event. Expensive does not mean safe, and imported does not mean up to standard. Buy a small pack and try it for 1 week. Look for a red bottom and marks from tightness, and stock up only if it fits.
- Cost
From 200 or 300 yuan a month up to more than 1000 yuan, depending on the grade you use.
- Benefit
No official body has published a ranking of "which brand is best," so this book does not recommend a brand. Only 2 kinds of information can be checked. The 1st is product-quality sampling notices from the State Administration for Market Regulation. The 2nd is a regulator's investigation of a specific event. After the "infant diaper formamide problem" drew attention in June 2026, the State Administration for Market Regulation led a joint investigation team to look into it.
- Notes
When the joint investigation team was set up, it only said "the relevant findings will be announced in due course." As of September 30, 2026, no conclusion of this investigation was found on the State Administration for Market Regulation website or in the national supervision sampling notices of recent years. How to buy in practice: expensive does not equal safe, and imported does not equal up to standard. Buy a small pack and try it for 1 week. Look for a red bottom and marks from tightness, and stock up only if it fits. Those sentences are the author's experience. The UK NHS says common reasons for a red bottom are urine and stool left on the skin too long, not changing often enough, and the diaper rubbing the skin. Some diapers that do not absorb, or do not breathe, can also cause it. The NHS also says the diaper should fit, and should not be wrapped too tight. How to look up sampling notices is in Section 5.

In a Scottish study, usually sleeping on a mattress another child had used carried about 3 times the sudden-death risk, but it is not clear whether the mattress caused it. A mattress is best bought new.
- Cost
No extra money. What you save is usually counted by the 1000 yuan. The hard part is holding back from kitting everything out all at 1 time.
- Benefit
Strollers, cribs, swaddles, and toys are all used for a short time, and the used market has far more supply than demand. Used goods have 3 safety lines. The 1st is the car seat. The US National Highway Traffic Safety Administration says a seat that has been in a moderate or severe crash must be replaced and never used again. All 5 conditions must be met before a crash counts as minor and the seat can stay in use. The 5 are: the car could be driven away on its own, the door nearest the seat was not damaged, no one in the car was injured, the airbag did not deploy, and no damage can be seen on the seat. A used seat can be used only after you confirm 5 things. The 5 are: it has not been in a crash worse than minor, the label shows the date of manufacture and the model, it has not been recalled, the parts are complete, and the instructions are still there. The date of manufacture and the model are how you check recalls and whether the seat is too old. The 2nd is the mattress. A Scottish study compared infants who died suddenly with healthy infants of the same age, and looked back at which mattress each slept on. Usually sleeping on a mattress another child had used carried a higher risk of sudden infant death syndrome (OR 3.07, about 3 times, 95% CI 1.51–6.22). When the mattress came from another home the figure was 4.78. When it came from the same home it was 1.64, and the latter does not count as a difference. The authors say the evidence is not yet enough to judge whether the mattress itself caused it. The 3rd is the crib. From June 28, 2011, the United States has enforced a new mandatory crib standard, and a used crib has to meet it too. The old style, with 1 side rail that drops, was phased out. The US Consumer Product Safety Commission says an old crib has to be taken apart and broken in the test before anyone knows whether it complies, so it gives no way to check it yourself. From November 2007 to April 2010, the United States reported 147 deaths linked to standard-size cribs. Of those, 35 were a problem in the crib's structure, and 18 were a failure of a rail that could be dropped. Loose screws caused at least 10 deaths. US law lists inclined infant sleepers as banned goods, no matter which year they were made, and that was written into the regulations in 2023 (United States, United Kingdom).
- Notes
The evidence grade stays C, because the money-saving 1/2 has no literature. The 3 safety lines each have a source, but all of them are from the United States and the United Kingdom. No official Chinese notice aimed at used cribs or used car seats was found. The mattress study was challenged: in 1997 a letter said family income and bed-sharing could mix up the effect of a used mattress. Why a car seat is used in the car is in Section 1, item 10 (car seat). How an infant sleeps, and what goes on the bed, is in item 1 of this section (sleep on the back). Postpartum centers, early-education classes, and baby swim classes are neither recommended nor ruled out in this book. Each of them is a large sum you can spend or not spend, and the 24-hour cooling-off period in Section 5 applies.

20.12 If the child has severe eczema or an egg allergy, do not avoid peanuts; add them early under a doctor's guidance, but never feed a whole peanut

- In plain terms
An infant with severe eczema or an egg allergy starts eating peanuts regularly at 4 to 11 months. Among those whose skin test showed no reaction at the start, 1.9% of the ones who ate peanuts had a peanut allergy at 5 years, against 13.7% of the ones who avoided peanuts completely. Among those whose skin test already showed a slight reaction, 10.6% of the ones who ate peanuts were allergic, against 35.3% of the ones who avoided them. See a doctor with the child before you add peanuts. Never feed a whole peanut. It can be inhaled into the airway.
- Cost
A jar of peanut butter is a few dozen yuan. Feeding it a few times a week is easy to fit in. Before you add it, take the child to a doctor 1 time for an assessment.
- Benefit
The UK LEAP randomized trial. 640 infants were enrolled, on the condition of severe eczema, an egg allergy, or both. They were 4 to 11 months old at enrollment. They were randomly split into 2 groups. 1 group ate peanut products regularly, and 1 group avoided them completely, and peanut allergy was checked at 60 months of age. Among the 530 whose peanut skin test was negative at enrollment, 13.7% of the avoidance group were allergic, against 1.9% of the group that ate peanuts (P<0.001). Among the 98 whose skin test was already weakly positive at enrollment, 35.3% of the avoidance group were allergic, against 10.6% of the group that ate peanuts (P=0.004). Serious adverse events did not differ between the 2 groups.
- Notes
The form can only be thinned peanut butter, or peanut flour mixed into complementary food. Never give a whole peanut. The Health Commission's core feeding messages say it plainly: "Whole peanuts, nuts, jelly, and similar foods are easily inhaled into the airway and cause choking, and infants and young children should not eat them." What to do if a child chokes: under 1 year of age, see Section 13, item 43 (an infant choking and unable to cry); at 1 year or older, see Section 13, item 26 (someone choking and unable to speak). This applies only to high-risk children: those with severe eczema, or those already allergic to egg. You must see a doctor for an assessment before adding peanuts. Do not try it at home on your own. The LEAP trial gave every child a skin test before enrollment. Children whose skin-test reaction was greater than 4 millimeters were kept out of the trial and were not given peanuts to eat. The evidence for ordinary children is much weaker. In another trial of 1162 ordinary breastfed infants, counted by the group they were assigned to, food allergy was 5.6% in the early-introduction group and 7.1% in the usual group, and the difference was not significant (P=0.32). It was significant only among the children who actually did it (2.4% against 7.3%). That way of counting tends to make the effect look larger. The authors themselves say the assigned-group count did not prove efficacy, but it does show that early introduction is safe. Official Chinese feeding documents currently say nothing on "introduce allergenic foods early, or avoid them," so this item follows the international trial evidence. The timing needs care too: LEAP started at 4 months, while in China complementary foods start at a full 6 months. See item 4 of this section (breast milk only for the first 6 months). For a high-risk child, at how many months to start, and how to start, follow the doctor.

20.13 Watch these newborn jaundice signals: yellow within 24 hours of birth, or cannot be woken and will not feed, go to the hospital that day; getting yellower, or pale stools, go and get it checked soon
- Money Low

- In plain terms
More than 8 in 10 newborns are a bit yellow. Yellow before 24 hours of age, or yellow together with being sleepier than usual and hard to wake, not feeding, or a body that is floppy or stiff, means go to the hospital immediately. After 24 hours, getting yellower, or stool that turns pale, also means get it checked soon. Pale stool can be biliary atresia (胆道闭锁). Bilirubin that is too high can injure the brain and leave a disability for life.
- Cost
Registration plus a bilirubin test, within a few hundred yuan. If phototherapy is needed, it is usually about 2 days in hospital. The hard part is making the trip even when the child still looks all right.
- Benefit
The American Academy of Pediatrics 2022 guideline says "more than 80% of newborns will have some degree of jaundice." Bilirubin that is too high can cause acute bilirubin encephalopathy and kernicterus (核黄疸). The guideline calls kernicterus "a permanent disabling neurological disease." Its features include cerebral palsy with limbs that twist on their own, an inability to look upward, poor enamel on the baby teeth, and hearing damage. The guideline lists "jaundice within 24 hours after birth" as a risk factor for going on to severe jaundice. The UK National Health Service (NHS) splits what it tells parents into 2 tiers. Tier 1 is to call emergency services or go straight to emergency care. That tier is a child who has jaundice and, at the same time, is sleepier than usual or hard to wake, is not feeding, is floppy or stiff, or has jerking movements. A child who is jaundiced before 24 hours of age is also in this tier. Tier 2 is to see a doctor as soon as possible. That tier is age past 24 hours, jaundice that is not getting better or is getting worse, urine that is dark yellow or brown, or stool that is pale cream. A global estimate is that in 2010 about 480000 newborns had Rh hemolytic disease (caused by a blood-type mismatch between mother and child) or extreme hyperbilirubinemia. Of those children, 24% died, and 13% developed kernicterus. Of the children who survived kernicterus, more than 83% were left with 1 or more impairments. Pale stool can be biliary atresia. That disease needs a Kasai operation (葛西手术) within 60 days before the result is clearly better. From 2004, Taiwan printed a stool color card in every newborn's child-health handbook. The share who had the operation within 60 days rose from 60% in 2004 to 74.3% in 2005. In another Taiwan-wide study, after the color card was introduced, mortality among children with biliary atresia fell from 26.2% to 15.9%. That is a before-and-after comparison, not a trial that split children into groups.
- Notes
Phototherapy (light treatment) uses a special light on the child's skin so bilirubin changes into a form that is easier to pass. Do not replace it by sitting the child in the sun at home. The American Academy of Pediatrics says sunshine is hard to make both safe and free of sunburn, so it does not recommend it as treatment. Do not feed water or glucose water to bring the yellow down. That guideline also says not to. Looking at how yellow the skin is does not estimate how high the bilirubin is. The UK NICE guideline states that you should not rely on the eye alone. If you are not sure, go and measure it 1 time. A child born before 38 weeks, a child whose older sibling had phototherapy, or a child who has not had enough breast milk is more likely to turn severely yellow. Follow-up follows the national basic public-health service specification: within 1 week after discharge, a health worker visits the home. At 28 to 30 days after birth, go to a community health service center or a township hospital for a follow-up visit. Both of the 2 visits check jaundice. If the doctor arranged a bilirubin recheck at discharge, go at the time that was set. If a full-term child is still yellow after 14 days, or a preterm child after 21 days, the NICE guideline requires 1 check of conjugated bilirubin, and a look at whether the stool is pale. Jaundice itself does not delay vaccination. See item 3 of this section (vaccines). Newborn screening is in Section 27, item 13 (heel-prick screening).

20.14 If you are eating vegetarian while pregnant or breastfeeding, take vitamin B12 or eat fortified foods; the child's complementary foods need animal foods, and if you insist on a fully vegan diet, see a doctor first
- Money Low

For Pregnant or planning / Child under 3 at home
- In plain terms
Vitamin B12 is found almost only in meat, eggs, and milk. Someone eating fully vegan has to rely on fortified foods or a supplement. If the mother eats fully vegan and does not supplement, a breastfed child is likely to run short. The trouble usually shows at 4 to 8 months of age: not growing, development falling behind, and, if it drags on, nerve damage that may not come back. The child's complementary foods need animal foods every day.
- Cost
1 bottle of vitamin B12 supplement is a few dozen yuan, 1 tablet a day. Adding egg, meat, or fish to the child's complementary foods does not cost much. The hard part is changing your mind when the whole family has decided that eating vegetarian is healthier.
- Benefit
In 2003 the US Centers for Disease Control and Prevention reported 2 children. Both were breastfed by mothers who ate vegetarian, both were short of vitamin B12, and both failed to grow and fell behind in development. The 1st child's mother had eaten fully vegan for 7 years. The child was admitted at 15 months of age, was found to have severe anemia, and a brain scan showed atrophy of the whole brain. After B12 was given the child improved, but at 32 months of age speech and language were still behind. The report says the most common reason an infant or young child is short of B12 is that the mother's diet is short of it. A breastfed child usually shows it at 4 to 8 months of age. If the shortage lasts long enough, some children are left with nerve damage that cannot be reversed. How bad it is depends on how severe the shortage was and how long it lasted. The only reliable sources that do not depend on fortification are animal foods: meat, dairy products, and eggs. Most of the B12 that occurs naturally in plants cannot be used by the body. Cereal, soy milk, and nutritional yeast with B12 added can be a steady source. The report asks people who eat vegetarian, especially women who are pregnant or breastfeeding, to find out how much B12 is in what they eat, or to ask a nutrition professional. The 2016 position paper of the Academy of Nutrition and Dietetics states: "People who eat fully vegan need a reliable source of vitamin B12, such as fortified foods or a supplement." The 2017 complementary-feeding position paper of the European Society for Paediatric Gastroenterology, Hepatology and Nutrition states that all infants should eat iron-rich complementary foods, including meat or iron-fortified foods. A fully vegan diet should be followed only under proper supervision by a doctor or a dietitian, and parents have to understand the serious consequences of not supplementing as advised. The National Health Commission's 2020 core feeding messages require no fewer than 4 kinds of complementary food a day. That includes at least 1 animal food, 1 vegetable, and 1 cereal or tuber food.
- Notes
Disputed. The 2016 position of the Academy of Nutrition and Dietetics is that a well-planned vegetarian diet and a well-planned vegan diet are suitable for every stage of life, including pregnancy, breastfeeding, infancy, and childhood. It also states that people who eat fully vegan need a reliable source of B12, such as fortified foods or a supplement. The 2 sides disagree on whether a fully vegan diet can feed an infant. They do not disagree that B12 must be supplemented. A vegetarian diet that includes eggs and milk is lower risk, but eating very little egg and milk can still leave a person short. The mother of the 2nd child in the report had eaten vegetarian for 20 years and almost never touched meat, fish, or dairy. Early symptoms of B12 shortage have no particular pattern. The US Centers for Disease Control and Prevention says anemia markers are not suitable for screening. If the child is growing slowly, or development is not keeping up, tell the doctor at the visit that the family eats vegetarian, and let the doctor decide what to test. Do not refuse treatment the doctor gives because you trust a folk remedy. See Section 16, item 4 (do not stop proper treatment to try a folk remedy). How to add complementary foods is in item 4 of this section (add complementary foods from 6 months). Folic acid in pregnancy is in Section 27, item 1 (supplement 0.4 milligrams of folic acid every day).

20.15 When buying things for an infant, do not buy a wheeled baby walker; if you want a place to sit and play, use a stationary activity center with no wheels, or a play yard
- Money No cost
- Time Minutes
- Willpower None
- Benefit Medium
- Measured in Lifespan
For Child under 3 at home / Pregnant or planning
- In plain terms
A child sitting in a baby walker can shoot 0.9 meters or more in 1 second, and most injuries happen while an adult is right there watching. From 1990 to 2014, children in the United States made about 230000 emergency visits because of baby walkers. 9 in 10 of the injuries were to the head and neck, mostly the child and the walker falling down stairs together. A baby walker also does not teach a child to walk.
- Cost
No money, and you save the price of 1 walker. A stationary activity center with no wheels, or a play yard, has to be bought separately, or it can be borrowed. The hard part is being willing to throw it out when an elder gave it, or when you already bought it.
- Benefit
From 1990 to 2014, children under 15 months who went to emergency care for a baby-walker injury made an estimated 230000 visits (exact value 230676). Of those, 90.6% were injured on the head or neck, and 74.1% fell down stairs while sitting in the walker. 4.5% were admitted after the injury, and 37.8% of the admitted children had a skull fracture. From 2010 the United States enforced a mandatory safety standard for baby walkers. Over the 4 years after that, the average number of injuries each year was 22.7% lower than over the 4 years before. The authors still say a baby walker remains an important source of injury to children, and it can be avoided. The American Academy of Pediatrics 2001 position statement says that from 1973 to 1998 the United States reported 34 deaths linked to baby walkers. Warning labels, publicity and education, an adult watching beside the child, and a stair gate have all been shown not to prevent this kind of injury. A baby walker also has no clear benefit, so the academy calls for a ban on making and selling wheeled baby walkers, and recommends a fixed activity center with no wheels instead. The note the academy gives parents says most baby-walker injuries happen while an adult is right there watching. A child sitting in a walker can shoot 3 feet (about 0.9 meters) or more in 1 second. On whether a walker helps a child learn to walk, the academy says it does not, and may delay it. The evidence that it delays walking is not strong. In 1 Irish survey of 190 infants, children who used a walker walked independently about 3 weeks later on average (61.12 weeks against 58.14 weeks). The more they used it, the later they walked. That was only a 1-time questionnaire, with no grouped comparison. 1 small review in 2002 found 2 trials that assigned children at random, and neither showed a clear effect on when walking started. Pooling 4 studies, the delay was 11 to 26 days, and the quality of the studies was low throughout.
- Notes
From April 7, 2004, Canada has banned the sale, advertising, and import of baby walkers. Used ones may not be sold at flea markets or garage sales either. For a walker already in the home, the official advice is to take it apart and throw it out. The current Canada Consumer Product Safety Act puts it on the banned-sale list. Nobody may make, import, advertise, or sell a walker. No ban was found in China. Infant baby walkers are made to the current national standard GB 14749-2006. The standard governs the product itself. It does not stop a child from shooting toward the stairs. From 1997, a baby walker in the United States either had to be made wider than a door, or had to have a brake at the edge of a step, and the academy still considers that not enough. A child sitting in a walker can reach higher, and can also pull a tablecloth and tip hot coffee, grab a pot handle on the stove, or fall, walker and all, into a pool or a bathtub. The academy suggests 3 substitutes: a fixed activity center with no wheels (the seat can turn and can bounce), a play yard, and a high chair. When you send the child to a childcare place or to someone else's home, confirm they are not using a baby walker there either. The order for buying big items is in item 11 of this section (consider big items in the order borrow, buy used, buy new).

20.16 From birth, give the child 400 international units of vitamin D every day, and especially do not skip it if the child is breastfed; a full-term baby generally does not need extra calcium in the first 6 months

Breast milk has very little vitamin D, and an infant should not be in the sun for long, so it has to be supplemented every day. In a rural area of Turkey, 3.8% of the children who were not supplemented got rickets within 1 year, and not 1 of the children who were supplemented got it. Give the amount the doctor says. A normal full-term child generally does not need extra calcium in the first 6 months.
- Cost
1 bottle of vitamin D drops is a few dozen yuan, given 1 time a day. The hard part is remembering every day, at least until 1 year of age.
- Benefit
A Cochrane review in 2020 says breast milk is low in vitamin D, and an infant who takes only breast milk is likely to fall short. The American Academy of Pediatrics says a child who stays outside for long needs sun protection, so every infant needs vitamin D. The National Health Commission's 2020 core feeding messages say: "From birth, an infant should receive 400-800 international units of vitamin D every day, under a doctor's guidance." The same passage also says: "A normal full-term infant generally does not need a calcium supplement in the 6 months after birth." The 2016 global consensus on rickets had 11 international academic bodies taking part. The consensus says 400 IU (10 micrograms) a day is enough to prevent rickets. Every infant should be supplemented from birth to 12 months, whether breastfed or formula-fed. That is a strong recommendation, and the evidence quality is moderate. In a study in rural northeastern Turkey, no child who received 400 IU a day got rickets. Among the children who were not supplemented, 3.8% newly developed rickets over 12 months. The consensus also says that, worldwide, there is no report of radiographically confirmed rickets in an infant who received 400 IU on time every day. In 1992 China ran a grouped study in 4 cities, north and south, supplementing 100, 200, or 400 IU a day. By 6 months, no child in any of the 3 groups had rickets. But vitamin D in the cord blood of northern newborns was only about 1/3 of the southern level (12.5 against 35.0 nmol/L). The more that was supplemented, the higher the level at 6 months, and the authors suggest 400 IU a day.
- Notes
The grade is B because the figures on preventing rickets come from comparisons that were not randomly assigned. A Cochrane review in 2020 pooled the trials that supplemented infants. At 400 IU a day, children short of vitamin D were fewer by about 4 in 10 (RR 0.57). The trials were all small, and they could not show whether rickets fell. The benefit is set at medium by judgment, because how common infant rickets is in China was not found. The advice for formula-fed infants is not the same everywhere. The American Academy of Pediatrics says that if the child drinks more than about 800 milliliters (27 ounces) a day of formula with vitamin D added, no extra supplement is needed. The domestic core messages say every infant is supplemented, and the exact amount follows what the doctor says. Do not raise the dose yourself. In a Canadian trial, the group on 1600 IU a day had vitamin D in the blood that was too high, and that arm was stopped early. Growth and bone did not differ across the groups from 400 to 1200 IU. An adult who is not short of vitamin D should not supplement it in order to live longer. See Section 6, item 3 (people who are not short of vitamin D).

20.17 When bathing an infant, put cold water in first and then mix in hot, and test it with the inside of the wrist or the elbow before putting the child in; if the water heater can set the outlet temperature, set it below 50 °C
- Money No cost
- Time Minutes
- Willpower Some
- Benefit Medium
- Measured in Lifespan
For Child under 3 at home / Pregnant or planning
- In plain terms
Hot water at 65 °C, in contact for 2 seconds, gives most adults a 3rd-degree burn, and an infant's skin is thinner. Put cold water in the basin first, then mix in hot. Stir it even, test it with the inside of the wrist or the elbow, and only then put the child in. Do not wash the child under a running tap. After 1 US state lowered the factory outlet temperature of water heaters to 49 °C, hospital admissions of children for hot-water burns fell by more than 1/2.
- Cost
No money. Each bath takes 1 or 2 extra minutes to test the water. If you want it steadier, you can buy a water thermometer. The hard part is still testing before you put the child in, even when the child is crying and the adult is in a hurry.
- Benefit
The US Consumer Product Safety Commission says most adults get a 3rd-degree burn from 2 seconds in water at 150 °F (about 65 °C). Water at 140 °F (60 °C) for 6 seconds, or water at 130 °F (about 54 °C) for 30 seconds, also scalds. Even at 120 °F (49 °C), a soak of 5 minutes can still cause a 3rd-degree burn. A World Health Organization report says the skin of infants and young children is thinner than an adult's, so a lower temperature can scald, and the scald goes deeper. The American Academy of Pediatrics teaches parents to test the water in the basin with the inside of the wrist or the elbow, and that it should be "warm, not hot." "Never bathe a baby under running water," because water moving through the pipes swings hot and cold, and a small change can scald. In 1983 the US state of Washington passed a law that new water heaters leave the factory preset at 49 °C. By 1988, the average home hot-water temperature had fallen from 61 °C in 1977 to 50 °C. Hospital admissions of children for tap-water burns fell from 5.5 a year in the 70s to 2.4 a year. Burned area, skin grafts, deaths, and days in hospital all fell too. That is a before-and-after comparison, not a trial that split people into groups. Australia sets a maximum of 50 °C for bathroom hot-water taps. In New South Wales from 1989 to 1996, the rate of hospital admission for burns among children aged 0 to 4 fell by 13%. Severe burns, those with a hospital stay of 10 days or more, fell by 30% in the last 2 years of the 2nd stage. In 3 Chinese hospital series of childhood burns, scalds from hot liquid accounted for 87.6% to 90.6%, and children 3 years or younger were the largest share.
- Notes
The grade is B because the figures come from before-and-after comparisons and from case records, with no trial that split people into groups. The benefit is set at medium by judgment, because a separate domestic figure for bath-water burns was not found. Water heaters come in 2 kinds. An instantaneous heater, such as a gas water heater, has its outlet temperature set directly below 50 °C. A storage electric water heater is a separate case. The World Health Organization guide on preventing Legionella suggests keeping the heater outlet at 60 °C, and hot water at the tap no lower than 50 °C. Legionella is a bacterium that can cause pneumonia. The same guide also says that preventing Legionella must not raise the risk of a child being scalded, and a thermostatic mixing valve can be fitted at the outlet. If none is fitted, rely on cold first, then hot, and on testing before you put the child in. Do not walk away during the bath. The American Academy of Pediatrics says more than 1/2 of children who drown in a bathtub are under 1 year of age. See Section 1, item 12 (stay in sight near water). Do not carry hot water or hot soup while you are holding the child. The World Health Organization says that in high-income countries, the most common burn in an infant under 1 year of age is a scald from a hot drink in a cup. If the child is burned, run cool water first. See Section 13, item 14 (run cool water after a burn).
