17 Elderly at home

Scope: money and personal freedom. When you have elderly family at home, most trouble is not running out of money. It is failing to talk things through and get papers signed in advance. Pressure ulcers from long-term bed rest, cataract surgery, fractures from an elder walking a dog, and a hospital check when memory gets worse: these four are counted in mortality terms and are not compared with the other entries in this section.

Items in this section (13)
  1. 17.1 While the elder is still lucid, name a future guardian in writing
  2. 17.2 Make a will, and remember that a later will replaces an earlier one; a notarized will no longer ranks ahead
  3. 17.3 Keep the elder's money in a separate account, and set a two-person rule for large spending
  4. 17.4 Give the elder a line they can always use: “I need to go home and talk with my family”
  5. 17.5 Do not touch any "invest in elder care" pitch that makes the elder pay first: a card, a bed, an elder apartment, travel elder care, and buying elder products are the same illegal fund-raising
  6. 17.6 Except insurers’ housing reverse mortgage insurance, avoid all other “housing-for-pension” pitches; never mortgage the home to buy wealth products
  7. 17.7 If an elder at home is bedbound long term or severely disabled, apply for long-term care insurance at the yibao office where they are enrolled; it is not paid only to the elderly
  8. 17.8 If someone at home is bedbound long term, treat pressure ulcers as the first enemy: an electric air mattress, turning on a schedule, and a daily look at the places where bone sticks out
  9. 17.9 If an elder's cataracts already affect seeing the way ahead, get an ophthalmology workup for surgery; if both eyes are affected, do not delay the second eye too long
  10. 17.10 When an elder walks the dog, hold the leash in the palm, do not wrap it around the hand, keep the leash short, and train the dog not to lunge forward on the leash
  11. 17.11 Before sending an elder to a nursing home, check that it is registered with the civil affairs bureau and sign a written agreement; the deposit is at most 12 months of bed fees and must go into a dedicated bank account
  12. 17.12 If a parent of an only child is in hospital, check whether your province's regulations give the child care leave; if they do, apply to the employer in writing with the hospital certificate
  13. 17.13 When an elder's memory keeps getting worse and daily life starts to slip, take them to a memory clinic or neurology for one check, and first rule out depression, medicines, and poor hearing or vision, causes that can be changed

17.1 While the elder is still lucid, name a future guardian in writing

Value: HighEvidence A
In plain terms

While the elder's mind is still clear, spend a few hundred yuan at a notary office on a written document that names who will be their guardian later. Without that document, once they cannot decide for themselves, the order set by law applies: spouse first, then parents and children, then other close relatives. If the family fights over it, the residents' committee (居委会), the villagers' committee (村委会), the civil affairs bureau (民政), or a court names the guardian.

Cost

A few hundred yuan at a notary office (公证处) for the document. A conversation with the elder can settle it. The hard part is bringing it up: you have to say, face to face, that a day will come when they can no longer manage for themselves.

Benefit

The Civil Code (民法典) says an adult who is clear-headed and can decide for themselves (in law, an adult with full capacity for civil conduct (具有完全民事行为能力的成年人)) may agree in advance who will be their guardian. That person can be a close relative, or another willing individual or organization. The agreement must name the guardian in writing (以书面形式确定自己的监护人). When they later cannot decide, or can decide only in part (lose or partly lose capacity for civil conduct (丧失或者部分丧失民事行为能力)), the person named in the document takes charge. Without that document, Article 28 of the Civil Code sets the guardian in this order: spouse; parents and children; other close relatives; then another willing individual or organization with the consent of the residents' committee, the villagers' committee, or the civil affairs bureau. Article 31 says that if people dispute who the guardian should be, the residents' committee, the villagers' committee, or the civil affairs bureau designates one. Anyone who disagrees may ask a court to designate the guardian, and they may also apply to a court directly.

Notes

This document is agreed guardianship (意定监护), meaning a person the elder names in advance. That person ranks ahead of the default legal order. Elders who live alone, whose children live far away, or whose family situation is complicated need this more. Notarizing it at a notary office is recommended.

The law recognizes 6 kinds of will, and each kind has steps that must be completed. The kind you write yourself must be written by the person from start to finish, signed, and dated with the year, month, and day. If several wills conflict, the last one controls. From 2021, a notarized will no longer automatically ranks ahead of the others.

Cost

Writing a will in your own hand costs nothing. Notarizing one at a notary office (公证处) costs a few hundred yuan to more than a thousand yuan. The hard part is bringing it up: you have to talk with the family about what happens after death.

Benefit

The Civil Code (民法典) recognizes 6 forms of will: written by the person, written by someone else at their request, printed, audio or video, oral, and notarized. Each has its own formal requirements (形式要件). For the kind written by the person, Article 1134 of the Civil Code requires that it be "written by the testator in their own hand, signed, and dated with the year, month, and day." If several wills are made and they conflict, the last one controls. From 2021, a notarized will no longer automatically ranks ahead.

Notes

A handwritten will must be written by the person from start to finish, signed, and dated with the year, month, and day. A printed will needs 2 or more witnesses. The testator and the witnesses must sign every page and write the year, month, and day. An heir, anyone the will gives property to, and anyone with an interest tied to those people cannot be a witness. The judicial interpretation names the interested persons it means: their creditors, their debtors, and people who are in business with them as partners. Whether a spouse, a child, or another close relative of those people counts, the statute and the interpretation do not say directly. The author suggests not asking them to witness, so the will is not fought over later. If the form is wrong, the whole will is void. That is the most common way these fail. After the person dies, whether a small deposit or a house must be notarized for inheritance is covered in section 25, item 12 (do not default to notarization when handling an inheritance).

17.3 Keep the elder's money in a separate account, and set a two-person rule for large spending

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Big
  • Measured in Money
Value: HighEvidence B

For Older person in your care / Age 60 or over

In plain terms

People who scam elders often start with small gifts and warm concern, build up trust, then take a large sum. They also often say "do not tell anyone." Agree on a rule in advance: spending above a set amount has to be mentioned to a specific person first. Then the family knows before the money goes out, and the decision to pay is pushed back.

Cost

No cost. Talk it over with the family once and set an amount. The hard part is how to bring it up so the elder does not feel you are guarding against them.

Benefit

In 2022 the Supreme People's Procuratorate (最高检) released typical elder-fraud cases. It said these crimes first use small favors and a show of family concern to win trust, then false claims and promises of high returns to take large sums. In one case, a company selling fake collectibles sent staff to clean for elders and sit and talk with them, and even took them as "godparents." When they sold the goods they sealed them in a black bag and told the elder not to open it, so the elder's family would not find out. The company took in more than 1,920,000 yuan. The boss was sentenced to 7 years and 6 months for fund-raising fraud. In 2023 the Beijing Local Financial Regulatory Bureau (北京市地方金融管理局) warned elders not to believe a scammer who says "do not tell anyone." When you are unsure, talk with family, children, and neighbors and friends you trust, and ask the family before you invest. The U.S. Federal Trade Commission totaled the reported losses of people aged 60 and over. In 2020 the total was about 600000000 US dollars. In 2024 it had risen to about 2400000000 US dollars. The rise was mainly cases in which a single loss exceeded 100000 US dollars. Agreeing in advance that "spending above a set amount has to be mentioned to a specific person first" lets the family know before the money goes out, and pushes that decision back.

Notes

No study has compared how much less fraud families see after they set this rule. The grade is B because the scam pattern and the advice to "talk with family more" come from the procuratorate and from financial regulators. The basis is the same kind as item 6 (housing-for-pension). A person who is worked up is easier to talk into something: see item 4 ("I need to go home and talk with my family"). General rules for avoiding fraud are in section 8 and section 13. Do not describe this as "we are afraid you will be scammed." Describe it as "the household keeps one set of accounts," which an elder accepts more easily. Do not hand all of an elder's money to the children to hold. That leads to a different kind of dispute.

In plain terms

Sales pitches and fraud rely on pressure on the spot, so you pay before you have time to think. Once emotions are stirred up, people are more eager to buy what exaggerated ads are selling, and elders are more eager to buy than younger people. Agree with the elder in advance on the line “I need to go home and talk with my family.” They can say it at any time, have a way to step back on the spot, and decide after they cool down at home.

Cost

No cost. One conversation, a few minutes.

Benefit

Pitches and fraud both lean on live pressure. The U.S. Federal Trade Commission anti-scam guide says scammers want action before you can think. On phone calls they tell you not to hang up so you cannot verify. The guide says tell someone—a friend, family member, or neighbor—before you act; saying it aloud may expose the scam. The former China Banking and Insurance Regulatory Commission (银保监会) risk notice in 2021 also told elders to consult family before buying wealth products. One experiment enrolled 116 people aged 65–85 and 115 aged 30–40. Researchers used a win/loss game to stir some people’s emotions, then showed exaggerated ads. Those whose emotions were stirred wanted to buy more. Regardless of arousal, elders wanted to buy more than younger adults. Authors advise waiting until emotions settle before spending. Another experiment enrolled people previously cheated by mail fraud, mostly older. Without prior warning, nearly half agreed outright to a simulated telemarketing scam call. With a reminder call two to four weeks ahead, clearly fewer agreed outright. Agreeing on “I need to go home and talk with my family” gives the elder an exit without deciding on the spot.

Notes

Grade B because neither experiment tested this exact sentence. One tested buying urge when emotional; one tested whether advance warning helps—no study compared fraud rates for elders who had this line ready. Use together with item 3 (keep the elder's money in a separate account). Do fall-prevention fixes at home too—see Section 1. The line is a face-saving exit, so they do not have to stand up to the pressure on the spot. It is not a way to guard against them. When surrounded by sales staff, the hardest thing is speaking up to refuse in front of a room, harder even than judging whether the pitch is true.

17.5 Do not touch any "invest in elder care" pitch that makes the elder pay first: a card, a bed, an elder apartment, travel elder care, and buying elder products are the same illegal fund-raising

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Big
  • Measured in Money
Value: HighEvidence A

For Older person in your care / Age 60 or over

In plain terms

Elder-care labels that make the elder pay first and promise a return are all the same scam. Common ones are buying a card, buying a bed, buying an elder apartment, travel elder care, and selling elder products at a meeting. Under the rules, if this money is lost, you can only bear it yourself. A reported case took 136,000,000 yuan by pre-booking beds in an elder apartment and harmed more than 2100 people.

Cost

No cost. Read the labels below to the elder once. It takes a few minutes. After that, refuse on the spot: do not go to the lecture, do not go on the tour of the site, and do not leave a phone number. The hard part is that if the elder has already gone several times and gotten to know the people, they are harder to talk back out of it.

Benefit

The Ministry of Civil Affairs (民政部) lists five forms. Recognizing the label is more reliable than recognizing the person. The first is borrowing the name of an elder-care service agency: there is no real operation, they rent a site for a short time, and they claim a cooperation agreement with an elder-care home so they can trade on its name. The second is in the name of providing "elder-care services": they sell a "VIP card," a "membership card," or a "prepaid card," and have people prepay "elder-care service fees." The service they promise clearly exceeds the beds they can supply. The third is in the name of investing in an "elder-care project." Common labels are buying a stake in a wellness and elder-care base, and selling a fictional elder apartment. There is also long-term rental of elder-care beds, and "time bank" mutual-aid elder care. The cover is sales that promise to return the principal, leaseback after the sale, and an agreed repurchase. The fourth is in the name of selling "elder products": repurchase of goods, consignment sale, consumption rebates, free checkups, gifts, conference marketing, wellness lectures, and expert free clinics. The fifth is in the name of enjoying "travel elder care": cheap or free travel, stored-value rebates, and points-based elder care. The money cannot be got back. The regulation says so. Article 25 of the Regulations on Prevention and Disposal of Illegal Fund-raising (防范和处置非法集资条例) says, "Losses suffered from taking part in illegal fund-raising shall be borne by the participants themselves." Lost money can only be borne by the person who put it in. In officially reported cases, Dai and others illegally raised 136,000,000 yuan in the name of pre-booking beds in an elder apartment, with more than 2100 victims. Dai was sentenced to 15 years for fund-raising fraud and fined 400,000 yuan. Cao, in the name of investing in the elder-care industry, committed fund-raising fraud of 13,207,000,000 yuan, involving more than 110,000 people. Cao was sentenced to life imprisonment, deprivation of political rights for life, and confiscation of all personal property. Conference sales of health products were prosecuted as fraud. Ni used free banquets, gifts, and a car to pick people up and drop them off, and pulled elders in to hear lectures. The lecturers used "a fake persona, fake cases, and an inflated price cut" to invent cancer-prevention and cancer-fighting effects. Elders were induced to buy "dihydroquercetin" (二氢槲皮素) at tens of times the cost price. Sales were more than 620,000 yuan. Ni was sentenced to 10 years and 3 months and fined 120,000 yuan (nationwide; the regulations took effect on 2021-05-01).

Notes

The judgment does not rest on "this place looks legitimate." It rests on "did they make you pay first, and did they promise a return." An elder-care home sells services. Once the talk adds investment, a rebate, or getting the principal back, do not trust it. An elder being taken to a lecture, a trip, or a free clinic is not itself illegal, and it is hard to stop. The step you can stop is the payment, used together with the two-person confirmation rule in item 3. If money has already been paid, immediately follow section 8, item 2 and call 110 or 96110 to ask for a payment stop. At the same time, report it to the local lead department for handling illegal fund-raising. Housing-for-pension is a different path: see item 6 (housing reverse mortgage insurance). The cap on deposits and membership fees at a legitimate nursing home, and the account that receives them, are in item 11 (a deposit of at most 12 months of bed fees). If a cheap tour took people into a shop and they bought something, see section 5, item 50 (a tour group taken into a shop).

17.6 Except insurers’ housing reverse mortgage insurance, avoid all other “housing-for-pension” pitches; never mortgage the home to buy wealth products

  • Money No cost
  • Time Minutes
  • Willpower Some
  • Benefit Big
  • Measured in Money
Value: HighEvidence B

For Older person in your care / Age 60 or over

In plain terms

The only legitimate housing-for-pension arrangement is insurers’ housing reverse mortgage insurance (住房反向抵押养老保险), and it is still a pilot. The “housing-for-pension” that scammers describe is unrelated to it: they trick the elder into mortgaging the house, then using the borrowed money to buy the wealth products they recommend. Some people did not even know the house had been mortgaged. In the end the house was gone and they still owed the loan.

Cost

No cost. Keep the property deed at home; do not hand it to anyone pitching “housing-for-pension” (以房养老). Show mortgage, guarantee, and loan contracts to your children before signing. Hard when door-to-door promoters are warm and keep coming back—the elder feels rude saying no.

Benefit

The former CBIRC Consumer Protection Bureau (银保监会消保局) separated real from fake. The real product is “housing reverse mortgage insurance for older persons.” Elders with full legal title mortgage the home to the insurer; they keep living in it, using it, and collecting rent. Sale or disposal needs the mortgagee (insurer) to agree. They receive pension payments under the contract until death. The business remains pilot-scale with few participants. Fraudsters’ “housing-for-pension” is “completely unrelated” to the state pilot and is “using national policy as cover to promote illegal fund-raising.” The method is tricking elders into a mortgage, then using the borrowed money to buy recommended products—“often a Ponzi scheme of paying old debts with new money.” The notice quotes consequences: “Some participants did not even know their property was mortgaged; they ultimately lost the house and still owed the loan.” It also gives yield lines: “If a wealth product promises returns above 6%, question it; above 8% is very dangerous; at 10% or more, prepare to lose all principal; ‘guaranteed principal with high yield’ is financial fraud.” The same notice requires careful contract signing and not signing blank contracts (nationwide, June 2021).

Notes

Grade B because these lines come from regulator risk notices and judgment, not research. The 6%, 8%, and 10% lines are consumer reference points, not statutory thresholds. CBIRC was merged into the National Administration of Financial Regulation (国家金融监督管理总局); the original page cannot be opened—provincial financial regulator reposts are cited. Wealth loss is secondary; after the home is mortgaged the elder still owes a loan—the loss far exceeds the principal. General rules on signing and blank contracts: Section 8, item 17. After fraud: payment stop, Section 8, item 2.

17.7 If an elder at home is bedbound long term or severely disabled, apply for long-term care insurance at the yibao office where they are enrolled; it is not paid only to the elderly

  • Money No cost
  • Time Hours
  • Willpower None
  • Benefit Big
  • Measured in Money
Value: HighEvidence A
In plain terms

Long-term care insurance pays care-service fees and generally does not pay cash directly. If someone at home is severely disabled and has been so for half a year or more, apply at the yibao office where they are enrolled. Care fees have no deductible (起付线), so reimbursement is not held back until a set amount has been paid first. Employee enrollees get about 70% back. Urban and rural resident enrollees get about 50% back. Bed fees and meal fees are not covered. The document sets no age floor. A young person with severe disability can apply too.

Cost

Submit an application at the yibao office where the person is enrolled (医保 — basic medical insurance). Then have a disability-level assessment. If they pass on the first assessment, the fund pays the assessment fee. You do not pay it yourself.

Benefit

Long-term care service fees that meet the rules have no deductible (起付线). A deductible is a sum you pay yourself before reimbursement starts. Here you do not pay that sum first. People enrolled under the employee policy are reimbursed by the fund at about 70%. People enrolled under the policy for urban and rural residents who are not employed are reimbursed at about 50%. The most that can be paid in a year (the annual payment cap (年度最高支付限额)) is no more than 50% of the previous year's per-capita disposable income of urban and rural residents in the local pooling area. The people who can receive benefits are those who are enrolled and paying as required, whose disability has generally lasted 6 months or more, and who pass an assessment after they apply. At the start, severe disability is covered first. The document sets no age floor. A person 18 or younger who is not working is enrolled with their parents and, if eligible, receives benefits at the urban and rural resident rate. The fund mainly pays care institutions and care workers. In principle it does not pay cash directly to the disabled person. Institutional bed fees, meal fees, and other charges that are not care are not paid by the fund.

Notes

Disability must be "long-term and continuous (generally 6 months or more)." A few weeks in hospital is not enough to apply. If the person was not enrolled when the local program started and enrolled later, or enrolled again after a gap in payment, there is a waiting period before benefits, in principle 6 months. Bed fees, meal fees, and other charges that are not care are not paid by long-term care insurance. Medical costs that yibao should pay are not paid by long-term care insurance either. Someone already receiving a work-injury living-care benefit does not receive this on top. This money and the care subsidy inside the two disability subsidies (两项补贴) are two procedures and two sums of money. They do not conflict. See section 7, item 8 (the two subsidies). Care at home and in the community gets a more favorable reimbursement rate. The person does not have to move into an institution. The program is rolling out place by place, and the disability assessment standards differ. Call 12393 first, or the medical insurance handler where the person is enrolled, and ask whether it has started locally and how to book an assessment. If the assessment fails, do not give up. Apply again after the disability gets worse.

17.8 If someone at home is bedbound long term, treat pressure ulcers as the first enemy: an electric air mattress, turning on a schedule, and a daily look at the places where bone sticks out

  • Money Low
  • Time Daily
  • Willpower A lot
  • Benefit Big
  • Measured in Lifespan
Value: OrdinaryEvidence B
In plain terms

For elders who already had a pressure ulcer before hip-fracture surgery, the chance of dying within 30 days was about 20% higher. Switching to an electric air mattress cut new pressure ulcers by about 60%. In an intensive care unit (ICU), using a sensor reminder so that turning on time actually happened cut them by about 70%, while laying the person face down produced far more pressure ulcers. Look once a day at the sacrum, heels, hips, and the shoulders and back. If you find red that does not fade when pressed, find a nurse or a doctor that same day.

Cost

A home electric alternating-pressure air mattress starts at a few hundred yuan. A turn takes only a few minutes. The hard part is turning every 2 to 3 hours, including at night. One caregiver cannot keep that up alone.

Benefit

19520 patients who had hip-fracture surgery (U.S. NSQIP, National Surgical Quality Improvement Program, 2016 to 2019). People who already had a pressure ulcer before surgery had a higher share of deaths within 30 days. After other factors were taken into account, the link still held (OR 1.2, a 21% increase in the odds, P=0.004). They were also more likely to have other problems: deep-vein thrombosis (OR 1.59), pneumonia (OR 1.39), and unplanned readmission (OR 1.43). The average hospital stay was also 0.4 days longer. 324 hospitalized elders (mean age 86) were followed for a median of 18 months. A pressure ulcer on its own predicted long-term death (HR 1.500, 95% CI 1.059 to 2.126, P=0.020). "On its own" means that after other factors are set aside, it still predicts. 65 randomized trials were pooled (a network meta-analysis). They compared a powered active air support surface with a standard hospital mattress (a standard hospital support surface). A powered active air support surface is the electric air-mattress type. People using one had a lower rate of pressure ulcers (RR 0.42, 95% CI 0.29 to 0.63, moderate certainty). A powered hybrid air support surface was RR 0.22 (0.07 to 0.66). A Cochrane review of 11 trials and 4462 people, updated in 2026, compared turning every 2 hours with turning every 4 hours. The difference was unclear (RR 1.05, 95% CI 0.79 to 1.39, very low certainty). Comparisons among other turning intervals were equally uncertain. In a trial of 1226 intensive-care patients, wearable sensors that reminded staff in real time and gave a visual alert made turning every 2 hours actually happen. The pressure-ulcer rate was lower than usual care (RR 0.28, 95% CI 0.10 to 0.75, moderate certainty). In a trial of 116 people, the prone group (lying face down) had a higher pressure-ulcer rate than the supine group (lying flat) (RR 4.55, 95% CI 2.31 to 8.98).

Notes

The grade is B for three reasons. First, the two studies of mortality only saw that people who already had pressure ulcers later did worse. That is not the same as showing that preventing pressure ulcers brings mortality down. Second, the randomized trials on turning intervals are very uncertain. Third, the air-mattress evidence is firmer, but the studies compared mattresses on hospital beds, and applying that to a mattress at home is an extrapolation. Looking every day at the sacrum, heels, hips, and the shoulders and back, for red that does not fade when pressed, is ordinary nursing practice. There is no separate study of it. Care that long-term care insurance can pay for is in item 7. If a leg suddenly swells, treat it as deep-vein thrombosis: section 13, item 11. The stretch of long bed rest after a fall or a serious injury is in section 1, item 34.

17.9 If an elder's cataracts already affect seeing the way ahead, get an ophthalmology workup for surgery; if both eyes are affected, do not delay the second eye too long

  • Money High
  • Time Hours
  • Willpower None
  • Benefit Medium
  • Measured in Lifespan
Value: OrdinaryEvidence BDisputed

For Older person in your care / Age 60 or over

In plain terms

In women aged 70+, having first-eye cataract surgery sooner cut falls over the next year by about 34%; fewer people fractured bones. The number who fell at least once did not drop. Second-eye surgery showed no clear fall difference. Operated patients had about 16% lower hip fracture over one year.

Cost

Accompany the elder to ophthalmology once; the doctor decides whether to operate. One eye per surgery; many hospitals do day surgery and discharge the same day. If both eyes have cataracts, operate in two separate procedures. Out-of-pocket cost depends on intraocular lens (IOL) choice and local basic medical insurance (医保) reimbursement—ask before surgery.

Benefit

Randomized trial of 306 women aged 70+ with cataracts. Half had first-eye surgery within about 4 weeks; half waited 12 months per usual care. Over one year, early surgery cut fall counts by about 34%. Rate ratio 0.66, 95% CI 0.45 to 0.96. At least one fall: surgery group 49%, wait group 45%—no reduction. Two or more falls: surgery 18%, wait 25%. Fractures: surgery group 4 (3%), wait group 12 (8%). Cochrane included this trial: first-eye surgery in women reduced fall counts; second-eye surgery did not. Second-eye randomized trial, 239 women aged 70+: surgery group fall rate 32% lower but difference may be chance (rate ratio 0.68, 95% CI 0.39 to 1.19). U.S. Medicare 5% sample: 1,113,640 cataract patients aged 65+ (2002–2009), observational only. Surgery associated with about 16% lower one-year hip fracture odds (OR 0.84, 95% CI 0.81 to 0.87). Absolute difference 0.20 percentage points—about 2 fewer cases per 1,000 people.

Notes

Disputed: pooled across two randomized trials, the number who fell at least once did not fall significantly (OR 0.81, 95% CI 0.55 to 1.17). Western Australia all-population hospital records: in the year after first-eye surgery, fall-injury admissions rose 27%; between the two surgeries such admissions were 2.14× the pre-surgery rate. Before–after designs cannot separate surgery from aging. Another Australian cohort found fall counts dropped most after second-eye surgery—so if both eyes have cataracts, do not delay the second eye too long. Between the two surgeries, be extra careful walking and on stairs. Cochrane also included a vision-intervention trial where fall counts rose about 57%. Surgery fixes clarity; fewer falls still need Section 1, item 13 (balance and leg strength). Whether bones survive a fall: Section 1, item 39 (bone density testing) and Section 1, item 40 (treat osteoporosis if found). National eye health planning states cataract restoration surgery is widely available at county level—not only big-city hospitals. Main beneficiaries: your parents and grandparents.

17.10 When an elder walks the dog, hold the leash in the palm, do not wrap it around the hand, keep the leash short, and train the dog not to lunge forward on the leash

  • Money Low
  • Time Hours
  • Willpower Some
  • Benefit Small
  • Measured in Lifespan
Value: OrdinaryEvidence B

For Older person in your care / Age 60 or over

In plain terms

In the United States, fractures among people aged 65 and over while walking a dog were about 1700 in 2004 and about 4400 in 2017. About 80% were women, nearly 30% needed a hospital stay, and by single body site hip fractures were the most common. At a hand-surgery clinic, about three-quarters of patients whose hands were injured by a dog leash were hurt because the leash was wrapped on the hand and the dog pulled. So hold the leash in the palm, keep it short, and train the dog not to lunge forward.

Cost

Holding the leash and keeping it short costs nothing. Walk with the elder and show them once. Training the dog not to lunge takes time. You can teach the dog yourself or hire a trainer. Hiring someone costs money, and prices differ a lot by place. The hard part is that the elder is used to a retractable leash and used to wrapping the leash around the wrist, and has to change that.

Benefit

U.S. emergency surveillance data, scaled up to the whole country, counted fractures in people aged 65 and over who fell while walking a dog on a leash. There were about 1671 in 2004 and about 4396 in 2017, and the rise was statistically significant. Among the people with fractures, 78.6% were women, and 28.7% were hospitalized. By single body site, the hip was the most common, at 17.3%. By broad region, the upper limb was the most common, at 52.1%. Another national emergency dataset covers 2001 to 2018. Injuries related to a dog leash were estimated at about 360000 visits (exact figure 356746). Of those, 193483 visits were injuries from being pulled by a dog, and 136767 visits were trips or entanglement in the leash. Injuries per million people rose from 25.4 to 105.5. A hand-surgery clinic saw 443 patients with hand injuries from a dog leash over 5 years. About three-quarters were injuries to the hand or fingers from the leash wrapping and the dog pulling (the abstract says 337 people, 75.9%). About a fifth needed surgery (96 people, 21.6%). The U.S. Centers for Disease Control and Prevention counted about 86629 fall-injury visits a year from 2001 to 2006 that were related to cats and dogs. Of those, 88.0% were related to dogs. Among the dog-related falls, 26% happened while walking the dog, and 21.2% were the dog pushing or pulling the person down.

Notes

Holding the leash in the palm and not wrapping it on the hand is advice from the doctors at that hand-surgery clinic. They state that this protects the fingers and the wrist. It does not stop a person being pulled down when the dog lunges. They also suggest keeping the leash short: on a short leash the dog cannot build speed, so the pull is lighter; on a long leash the line also catches on things and trips people. A retractable leash let all the way out is a long leash. No study has compared retractable leashes with ordinary leashes for which one leads to more falls. A 2024 review states that there are no data on what kind of leash was in use at the time of injury. Training the dog not to lunge forward on the leash is mentioned both by the U.S. Centers for Disease Control and Prevention and by the authors of the fracture paper. That fracture paper also suggests that an elder who is thinking of getting a dog pick a smaller dog. No study has calculated how many fewer falls these steps produce, so the benefit magnitude is judged small. Whether the bones can take a fall: section 1, item 39 (a bone-density test) and section 1, item 40 (if osteoporosis is found, treat it). Balance practice: section 1, item 13 (balance and leg strength). Whether walking the dog counts as exercise: section 2, item 44 (count dog walking toward the walking total). The people who benefit are mainly your parents and grandparents.

17.11 Before sending an elder to a nursing home, check that it is registered with the civil affairs bureau and sign a written agreement; the deposit is at most 12 months of bed fees and must go into a dedicated bank account

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Big
  • Measured in Money
Value: Very highEvidence A

For Older person in your care

In plain terms

A legitimate nursing home has to register with the civil affairs bureau and sign a written agreement with you. The deposit is at most 12 months of bed fees, and service fees may be collected at most 12 months ahead. The deposit and membership fees must go into a dedicated account the home holds at a bank. If they go over the cap, ask you to transfer to a personal account, or give a receipt and no invoice, do not pay yet.

Cost

No cost. Before signing, call the local county civil affairs bureau (民政), or ask the nursing home for its registration receipt. Then compare the agreement, the fee sheet, and the account that receives the money. Half a day is enough. The hard part is that when beds are scarce, the family feels awkward picking at the contract.

Benefit

The Ministry of Civil Affairs (民政部) measures for the administration of elder-care institutions say that within 10 working days after a home takes an elder in, it must register with the civil affairs bureau. If the papers are complete, the bureau issues a registration receipt. The home must sign a service agreement with the elder or their agent, stating the care level, the fee schedule, refunds, and liability for breach. If they do not sign an agreement, the bureau orders correction and issues a warning; in a serious case the fine is up to 30,000 yuan. A 2024 document from 7 departments, including the Ministry of Civil Affairs, set caps on prepaid fees. Elder-care service fees may be collected at most 12 months ahead. The deposit is at most 12 times that elder's monthly bed fee. The deposit may be used only for refunds, advancing the cost of sudden medical care, offsetting unpaid fees, and compensation. Public homes, and homes that were built with public funds and are privately run, may not charge a membership fee. A home that is not yet built, or that cannot yet take elders in, may not charge a membership fee either. The deposit and membership fees must all be placed in a dedicated deposit account under third-party custody at a bank. They may not be collected into a personal account. When money is received, an invoice must be issued. A receipt may not stand in for an invoice. If the elder cancels before moving in, the home must return the prepaid money promptly. If the elder has moved in, deduct what has already been used and, in principle, refund the rest in one payment through the original channel. This document took effect on 2024-10-01. Provinces may set stricter rules. The 2025 custody guidelines require that after the home applies for a refund, the custodian bank pay it the same day, or the next day at the latest, in one payment back through the original channel.

Notes

A province may set a shorter cap on the deposit and a shorter period for prepaying service fees. Check that province's rules before signing. The agreement can follow the model elder-care service contract issued by that province's civil affairs bureau and market supervision administration. The model-contract library of the State Administration for Market Regulation (市场监管总局) has them. Registration is done after the home starts taking elders in. A home that just opened may still be filing. Ask when it will be registered. If a refund cannot be agreed, the document's path is negotiation, mediation, a complaint, arbitration, or a lawsuit. A complaint goes to the civil affairs bureau that supervises the home, and it is free. If you go to court, an ordinary first-instance procedure is required by the rules to finish within 6 months, and may be extended; you pay the lawyer yourself. If they promised, when taking the money, to return principal with interest or to pay a return, that is no longer ordinary prepayment. See item 5 (investing in elder care). The people who benefit are mainly your parents and grandparents. The prepaid-fee caps are written from the documents as of 2026-10.

17.12 If a parent of an only child is in hospital, check whether your province's regulations give the child care leave; if they do, apply to the employer in writing with the hospital certificate

  • Money No cost
  • Time Minutes
  • Willpower None
  • Benefit Medium
  • Measured in Money
Value: HighEvidence B

For Older person in your care

In plain terms

Some provinces set care leave so an only child can look after a parent in hospital, and the number of days differs by province. Henan is at least 20 days a year. Fujian is at most 10 days. Guangdong, when the parent is in hospital, is at most 15 days. Henan states that the leave counts as attendance. Fujian states that wages and benefits stay the same. If the employer refuses or docks pay, Fujian and Guangdong state that you can go to the human resources and social security department.

Cost

No cost. Search the national database of laws and regulations for your province's two ordinances. It takes a little over 10 minutes. Then submit a written application to the employer with the hospital certificate. The hard part is that the employer does not want to give the leave, and asking feels like it will offend the boss.

Benefit

Provinces put care leave in different local regulations. The Henan Provincial Regulations on the Protection of the Rights and Interests of the Elderly say that when an elder who holds the honorary certificate for parents of an only child (独生子女父母光荣证) is in hospital, the child's employer must give care leave. The leave is at least 20 days a year in total, and it counts as attendance. The Fujian Provincial Regulations on the Protection of the Rights and Interests of the Elderly say that when a parent of an only child is 60 or older and is in hospital with an illness, the employer must give the child time for care. That time is at most 10 days a year in total, and wages and benefits stay the same. If the employer does not pay wages and benefits for the care period, the human resources and social security department (人社) or the relevant department orders payment by a deadline. The Guangdong Provincial Regulations on Population and Family Planning cover couples with Guangdong hukou (户口 — household registration) who, in the years when the state encouraged one couple to have one child, voluntarily had only one child. When they turn 60, the employer gives the child 5 days of care leave a year. If they are ill and in hospital, the total is at most 15 days a year. If the employer does not provide the leave the regulations require, the human resources and social security department at county level or above orders correction. The liability chapter of the Henan regulations does not say what happens if the employer does not give the care leave.

Notes

To check your province: on the National Laws and Regulations Database (flk.npc.gov.cn), search the province name plus the regulations on the protection of the rights and interests of the elderly (老年人权益保障条例), and the province name plus the population and family planning regulations (人口与计划生育条例). Read both. Read only the version marked "in force" (有效). If the province has not written it, search the city's regulations. Provinces give this only to an only child. Someone with brothers or sisters is mostly outside the text. If the employer will not give the leave or has docked wages, call 12333 or file a labor-inspection complaint at the human resources and social security bureau (人社局). It is free. To get the docked money back, you can also apply for labor arbitration. It is free. The case is closed within 45 days after acceptance, and may be extended by at most another 15 days. Henan's text says "at least" 20 days. Fujian and Guangdong say "at most." Giving fewer days in Fujian or Guangdong is not a violation. The grade is B because each province sets the number of days and the conditions. The people who benefit are your parents. The money falls on you. The day counts for the three provinces are taken from the regulations as of 2026-10 and may be revised later.

17.13 When an elder's memory keeps getting worse and daily life starts to slip, take them to a memory clinic or neurology for one check, and first rule out depression, medicines, and poor hearing or vision, causes that can be changed

  • Money Low
  • Time Hours
  • Willpower None
  • Benefit Small
  • Measured in Lifespan
Value: OrdinaryEvidence B

For Older person in your care / Age 60 or over

In plain terms

Memory that keeps getting worse is not necessarily just muddled old age. First check for depression, medicines that do not suit them, and poor hearing or vision, causes that can be changed. Very few cases can actually be cured. Among people with dementia, those who truly improve are under 1%. Checking early has another use: while the elder can still decide for themselves, settle who will manage their money and their medical care later.

Cost

A visit to a memory clinic or neurology, with a cognitive test, a blood draw, and a urine test, takes half a day to a day. Fees follow the local schedule. Yibao (医保 — basic medical insurance) reimburses according to the rules. The hard part is that the elder does not admit they are confused and does not want to go.

Benefit

The UK NICE dementia guideline (NG97) requires that, when dementia is suspected, the history be taken from the person and from family who know them. It also requires a physical exam, blood and urine tests to rule out causes that can be reversed, and a cognitive test. The guideline names four things to check first: delirium (sudden confusion that comes in spells), depression, poor vision or hearing, and medicines with a heavy anticholinergic load. If those are ruled out and dementia is still suspected, refer the person to a specialist service such as a memory clinic for a diagnosis. A 2003 meta-analysis pooled 39 studies, of which 5620 people had a confirmed dementia diagnosis. Alzheimer disease accounted for 56.3%, and vascular disease for 20.3%. Causes that might be reversed accounted for 9%. Those who truly improved were only 0.6%: partial improvement 0.29%, full improvement 0.31%. The same NICE guideline requires an early discussion with the patient and the family about arrangements made in advance. That includes authorizing someone else to manage health and money, and the person's wishes for future care. A 2024 action plan by 15 departments, including the National Health Commission (国家卫健委), calls for pushing a first cognitive screen for people aged 65 and over, and encourages hospitals to set up memory clinics.

Notes

The benefit magnitude is set to small, because very little that is found can truly be reversed, and no study shows that checking early makes a person live longer. The grade is B because the workup comes from a guideline and the figures only show the share that can be reversed. The studies in that meta-analysis run through 2002. The share today may be lower. If a medicine is making the person confused, do not stop it on your own. Take the medicine box and ask the doctor. Becoming sluggish and unsteady on the feet a few weeks after a knock on the head is a different matter: section 13, item 10 (a head CT, computed tomography, after a knock on the head). For hearing loss, fit a hearing aid: section 33, item 17 (hearing aids). While the elder is still lucid, do item 1 (name a future guardian in writing) and item 2 (the will). If they can no longer manage their own affairs, apply to a court for a finding: section 33, item 18 (apply to a court for a finding). The people who benefit are mainly your parents and grandparents.
